Alternative Sentencing And Probation | Geauga County Court Of Common Pleas - General Division

Provides formal supervision for those awaiting trial or on community control (probation) as ordered by the court. Supervises probationers and monitors compliance to the conditions of their probation as well as providing other services such as referrals for social services. Provides specialized probation services for those needing intensive supervision such as those with mental illness, developmental disabilities, and sex offenders. Places offenders into drug treatment when needed. Provides case management for those in drug treatment and acts as a liaison between the court, client, and treatment facility. Also supervises persons in the work release and home detention programs.

Offers one-on-one assistance to help individuals determine which private, public, state and local benefits they may be eligible to receive. Includes benefits such as Medicaid, Medicare Part D, SSI, SSD, SNAP (food stamps), HEAP (LIHEAP), Golden Buckeye Card, Exemption Prescription Assistance programs, housing and others. Will assist clients with filling out and submitting applications and forms and provides follow-up with client. Visits seniors and adults with disabilities in the home, where they provide client with combined utility assistance application, assist client, to fill it out, mail it, and follow up as needed regarding LIHEAP. Makes presentations to senior groups regarding benefit programs and provides assistance completing applications.

Supported Living Waivers - Developmental Disabilities: Individual Options (IO), Level 1, Self Waiver | Pioneer Center

Provides supports needed by client (based on assessed needs) to continue living in the community. These waivers can be used to pay for a wide range of services to pay for housing, in-home services (such as help with meal preparation and household chores) and other services to maintain the independence of the client (such as transportation, help with budgeting, and life skills training as needed). There are three types of waivers depending on the level of the client's needs. The Individual Options (IO) Waiver is for people who may need a lot of help in their home and who may need many different kinds of services. The Level One Waiver is for people who do not need a lot of paid staff to provide services. Usually people using a Level One Waiver only need help with a few things or they have people who can help them with some of the things they need, like cooking meals, taking showers, or driving places. The Self-Empowered Life Funding (SELF) Waiver is for people who want to be in charge of some of their services. Those with a SELF Waiver can be in charge of hiring and training the people that provide services and manage their own budget for services. The board can help the individual select which waiver is right for them, and can help the client locate service providers who can accept the waiver.

Adult Services | Pioneer Center

Works to help adults with developmental disabilities achieve the highest level of independent function possible. Each new client receives an individual plan which is reviewed as needed. Services include work evaluation, basic skills development, recreational activities for seniors no longer working. A number of supportive services are provided to participants, including communication skills training, recreation and activities, social work, physical therapy and occupational therapy.

Safety Kits - Wandering Children/autism/parking Lot Safety | Connecting For Kids

Provides two kits designed to introduce safety around moving vehicles in parking lots and to intervene in wandering behavior typical of autistic children. Kit usually includes door and window alarms, an ID bracelet, visual stop signs and a guide on what to do about wandering behavior. Parking lot safety kit includes parent/caregiver safety guide and vehicle magnet to remind children how to be safe in parking lots.

Provides guidance to families of individuals with developmental disabilities to help resolve issues and navigate the developmental disability system. Offers support and resources to both individuals and family members. Assists with advocacy, planning, and support. NOTE: Service is not limited to school advocacy.
