Family Selected Service | Carroll County Board Of Developmental Disabilities

Assists families with the care of individuals with developmental disabilities who reside in the family home by providing limited funding for respite services, adaptive equipment and other expenses associated with meeting the needs of their family members with developmental disabilities. Items and services available include respite care, adaptive equipment, home modifications, special diets and other services such as individualized as counseling, sign language, books, classes, reimbursement for mileage to doctors, hospital, therapy visits, and more.

School Of Dancing Wheels | Dancing Wheels Company And School

Offers modern dance classes for youth and adults with or without disabilities, including wheelchair users. Includes classes such as ballet, jazz, hip hop, modern dance, dance technique, and wheelchair techniques. Holds summer dance workshops and a theater arts camp for youth. Provides classes tailored for persons who share the same disabilities, such as developmentally disabled older adults.

Personal Advocacy Program | Ardmore Inc

Acts as a surrogate family member to ensure that the participant’s rights are not violated. "A Life Plan" is developed to help provide guidance to the Advocate as to what is important to know about the person served. Provides participants an opportunity to socialize one to one with the Advocate and/or in the community in meaningful recreation and leisure activities such as swimming, hiking, meals out, concerts, sporting events, vacations, and church programs. Advocates can participate in ISP or special meetings. Help ensure participants are living and working in appropriate settings.

Supported Living Waivers - Developmental Disabilities: Individual Options (IO), Level 1, Self Waiver | Geauga County Board Of Developmental Disabilities

Provides supports needed by client (based on assessed needs) to continue living in the community. These waivers can be used to pay for a wide range of services to pay for housing, in-home services (such as help with meal preparation and household chores) and other services to maintain the independence of the client (such as transportation, help with budgeting, and life skills training as needed). There are three types of waivers depending on the level of the client's needs. The Individual Options (IO) Waiver is for people who may need a lot of help in their home and who may need many different kinds of services. The Level One Waiver is for people who do not need a lot of paid staff to provide services. Usually people using a Level One Waiver only need help with a few things, or they have people who can help them with some of the things they need, like cooking meals, taking showers, or driving places. The Self-Empowered Life Funding (SELF) Waiver is for people who want to be in charge of some of their services. Those with a SELF Waiver can be in charge of hiring and training the people that provide services and manage their own budget for services. The board can help the individual select which waiver is right for them, and can help the client locate service providers who can accept the waiver.

Works to help adults with developmental disabilities achieve the highest level of independent function possible. Each new client receives an individual plan which is reviewed as needed. Vocational Rehabilitation | Opportunities For Ohioans With Disabilities

Provides services for clients to find gainful employment or training. Specific services include: counseling and planning, personal and work adjustment training, vocational training, training supplies and books, financial aid to attend post-secondary classes, artificial limbs, braces, eyeglasses, hearing aids, psychotherapy, physical therapy, wheelchairs (if needed to work), occupational supplies, placement, small business development, and follow-up on the job. Also helps disabled students who are transitioning from school to work or college after high school. Some of the above services are provided by other agencies through a referral process.
