Client must have a primary diagnosis of a developmental disability, and must be age 16 or older. Beyond this, eligibility varies greatly depending on individual situation. Client must live in Cuyahoga or Lake County.
Current medical, dental, psychological, educational and vocational records. A social security history. Current residential history if coming from another residential facility.
24 hours a day, 7 days a week.

Generally third party pay. Fee arrangements depend on funding source. Clients may be private pay, but there are also various government sources.

Provides homes in which individuals with developmental disabilities receive 24 hour care and develop daily living skills which enable them to lead increasingly independent lives. Each person has an individualized plan with goals, and some eventually move into supported or independent living. There are a number of homes in Cuyahoga County, most of which can house up to 8 people.
