Provides speakers on issues related to caring for older adults. Sessions are approximately one hour, with 30 minutes dedicated to speaker time and 30 minutes for questions. Some commonly requested topics include the following: Demystifying Older Adult Services, Care and Communication of Those Living with Alzheimer's/Dementia, The Importance of Faith in Senior Care, and Adult Day Programs: Supporting In-home Caregivers and Introduction to the Sacred Art of Living and Dying.

Passport Home Care | Western Reserve Area Agency On Aging

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Supportive services division for Trumbull’s retired and elderly population provides social work and counseling, prescription assistance and Medicaid/Medicare counsel, emergency assistance program, and free income tax assistance through AARP during the months of February through April each year. Provides a variety of social and recreational activities, transportation, and social services for seniors. Also, provides outreach services, including assistance with filling out forms. On site the Center offers Blood Pressure Checks, legal counseling, and a benefit bank. Social and recreational activities may include social meetings, craft classes, line dancing, cards, trips, yoga and exercise classes. Offers information and referral to governmental and community resources.

Aging And Disability Resource Center (ADRC) | United Way Services Of Geauga County

Consults with older adults and adults with disabilities about their needs and preferences and then screens for a wide range of appropriate government benefits and other community services that they may be eligible for. Will assist the consumer with gathering any necessary documents and will guide them through applications process (including assistance with filling out forms) as needed. Agency provides follow-up with the client. Goal is to minimize the number of calls and contacts the consumer has to make, and encourage the use of the least costly and least restrictive services.

Provides free legal assistance via phone regarding problems that affect older adults living in Ohio. Subject areas include civil law issues such as consumer problems, Medicaid, Medicare, Social Security, subsidized housing, wills and estates. If the legal problem cannot be solved over the phone, the client may be referred to a private attorney who can provide the service. Also offers pamphlets on a variety of legal topics.
