Aging And Disability Resource Center (ADRC) | Catholic Social Services Of The Miami Valley

Refers individuals with aging-related or disability-related issues to appropriate available services in the community to ensure that individuals' needs are fully met. Helps individuals find help for housing, transportation, in-home care, long term care, and other health and wellness services, including benefits screening.

Aging And Disability Resource Center (ADRC) | Cuyahoga County Division Of Senior And Adult Services

Provides one-on-one information and assistance to aging adults and individuals with disabilities to help them identify their needs and preferences, and access community resources, benefit programs and other available social, medical and residential services. Also helps ensure that individuals and their caregivers are aware of the various living options, care options, and supportive services available to them.

Assists and connects older adults to in-home and community-based services to help foster and maintain their independence. Services include personal care and homemaking. No more than six hours of personal care/homemaking assistance provided per week.

University Center On Aging And Health | Case Western Reserve University

Provides a University-wide, interdisciplinary Center that offers numerous continuing education opportunities focusing on aging and health issues for professionals and the public. Programming includes workshops, conferences, and seminars throughout the year. All provide professional CEU's to health and social work practitioners. Also offers a graduate level certificate in Gerontology. The Center also participates collaboratively in Research efforts about Aging and Health with other University Departments/Schools. Also offers a certificate program in Gerontology.

Provides searchable databases that offer help locating organizations and agencies that serve older adults and their caregivers. Includes information about healthcare providers, health plans, prescription drug plans, medigap plans, hospitals, nursing homes, home care, long term care planning, medical equipment suppliers, and dialysis facilities. Some of the databases offer the ability to compare quality between providers.

Pre-Admission Screening System Providing Options And Resources Today (PASSPORT) | Catholic Social Services Of The Miami Valley

Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.
