Provides a wide variety of activities, including gentle exercise classes (such as tai chi, yoga, chair volleyball and stretch & strength), group outings and trips, open art studio sessions, painting classes, bell choir, holiday parties, bingo, trivia, educational sessions, games and arts & crafts.

Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides a variety of recreational and personal enrichment activities for seniors. Activities may include, but not limited to, cards, bingo, outings and trips, exercise classes, book discussion groups, movies, Tai Chi, yoga, computer classes, guest speakers, and parties.

Provides analysis of needs to those who are isolated and in need of help with age-related issues.

Refers individuals with aging-related or disability-related issues to appropriate available services in the community to ensure that individuals' needs are fully met. Helps individuals find help for housing, transportation, in-home care, long-term care, and other health and wellness services including benefits screening.

Provides speakers on topics related to health and safety to community groups of 10 or more. Has over 24 topics available. Presentations are typically an hour. Early requests strongly recommended.

Provides information and referral of social services and benefits information to older adults. Will provide assessments in the home for residents when needed. Offers counseling to help seniors cope with life-style changes, personal, marital, and family problems. Assist with forms completion and maintains a registry of chore service providers

Assists and connects older adults to in-home and community-based services to help foster and maintain their independence. Services include personal care and homemaking. No more than six hours of personal care/homemaking assistance provided per week.

Helps individuals and their families explore options and develop plans to care for older adults. Program helps to identify a loved one’s strengths, needs, concerns and preferences; to establish goals and implement a plan of care; to connect individuals to community resources and government benefit programs; to coordinate doctor appointments, transportation, and caregiver services. Services include counseling to help cope with issues associated with aging, such as changes in independence, health problems, dementia, death of a loved one, isolation, and family dynamics. Counseling is also available for coping with mental health issues, such as anxiety and depression.

Provide senior adults with daily recreational activities, educational events, lunch, socialization, and fitness programs. Supportive services division for Trumbull’s retired and elderly population provides social work and counseling, prescription assistance and Medicaid/Medicare counsel, emergency assistance program, and free income tax assistance through AARP during the months of February through April each year. Provides outreach to people experiencing mood disorders due to issues related to aging, psychological problems, and other issues affected by the aging process. Services include geriatric assessment, case management, counseling (group and individual), medication monitoring, in-home safety assessments and other services determined by case managers. Services to women include attention to psychiatric issues related to physical stages in their lives.

Guardianship program for severely mentally disabled persons who are identified by the mental health/developmental disabled systems for services. All referrals must go through clinical issues meetings or the case manager supervisor.

Provides outreach services in the home for seniors. Screenings such as blood pressure, cholesterol, hearing and vision, and appointments with a podiatrist are periodically available. Helps seniors complete various forms such as Medicaid forms. Offers a bereavement support group and a Parkinson's disease support group.

Consults with older adults and adults with disabilities about their needs and preferences and then screens for a wide range of appropriate government benefits and other community services that they may be eligible for. Will assist the consumer with gathering any necessary documents and will guide them through applications process (including assistance with filling out forms) as needed. Agency provides follow-up with the client. Goal is to minimize the number of calls and contacts the consumer has to make, and encourage the use of the least costly and least restrictive services.

Publishes and distributes "The Senior Living Guide" - a quarterly directory of long term care facilities and services in Cuyahoga and neighboring counties for older adults. Publication is also designed to assist professionals who counsel individuals considering care options. Also, maintains a website that provides the "Senior Living Guide."

Provides a variety of social, educational, and recreational activities for seniors. Includes some day trips, multi-day trips, and cultural events.

Offers information related to older adults and aging such as physical health, aging in place, fall prevention, caregiver support, financial well-being, and benefits and financial assistance programs.

Helps disabled older adults remain safe and independent at home. Prevents unnecessary nursing home placements. Consumer-Directed option gives consumers the option to serve as employer by hiring eligible individuals of their choice to provide their personal care services.

Provides recreational activities such as exercise classes, dancing, crafts, games, speakers, lunches, trips, outings, wellness programs, and computer classes for seniors.

Conducts gerontological research and provides data on older adults to legislators, researchers and the public. Features information affecting those age 50 and older.
