Western Reserve Geriatric Education Center | Case Western Reserve University

Provides educational programming, training, and consultation about gerontology and geriatric issues to faculty, health care professionals and consumers throughout the State of Ohio in order to improve health care for older adults. The Center is a consortium of nationally recognized academic leaders in gerontology and geriatric medicine. In addition to CWRU School of Medicine, the consortium includes Bowling Green State University, Miami University, Ohio University of Osteopathic Medicine and Northeastern Ohio Universities College of Medicine. Also makes available training modules/curriculum about a variety of topics related to aging, especially Alzheimer's disease.

Passport Home Care | Western Reserve Area Agency On Aging

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.
