Assists people who are eligible for nursing home level of care, remain in the community. They provide home health care, home delivered meals, physical therapy, in home assistance, adult day and respite care programs and transportation for errands or medical appoinments.

Guardianship program for severely mentally disabled persons who are identified by the mental health/developmental disabled systems for services. All referrals must go through clinical issues meetings or the case manager supervisor.

Provides free legal assistance via phone regarding problems that affect older adults living in Ohio. Subject areas include civil law issues such as consumer problems, Medicaid, Medicare, Social Security, subsidized housing, wills and estates. If the legal problem cannot be solved over the phone, the client may be referred to a private attorney who can provide the service. Also offers pamphlets on a variety of legal topics.

Provides a wide range of programs and trips. Some of the activities include luncheons, book discussions, current events discussions, exercise, knitting, bridge, Tai Chi, card games, films, and special events.

Provides a variety of recreational and personal enrichment activities for seniors. Activities may include, but not limited to, cards, bingo, outings and trips, exercise classes, book discussion groups, movies, Tai Chi, yoga, computer classes, guest speakers, and parties.

Provides searchable databases that offer help locating organizations and agencies that serve older adults and their caregivers. Includes information about healthcare providers, health plans, prescription drug plans, medigap plans, hospitals, nursing homes, home care, long term care planning, medical equipment suppliers, and dialysis facilities. Some of the databases offer the ability to compare quality between providers.

Provides information and assistance to aging adults and individuals with disabilities to help them identify their needs and preferences, and access community resources, benefit programs and other available social, medical and residential services. Also helps ensure that individuals and their caregivers are aware of the various living options, care options, and supportive services available to them.

Provides a wide variety of activities, including gentle exercise classes (such as tai chi, yoga, chair volleyball and stretch & strength), group outings and trips, open art studio sessions, painting classes, bell choir, holiday parties, bingo, trivia, educational sessions, games and arts & crafts.

Provides assistance with Medicare consultation and insurance information, agency referrals, home visits for Home Delivered Meal client assessment, educational and health-related programs, non-medical home care agency listings, community resource lists and caregiver consultation and education.

Provides specialized information and referral service to help clients seeking resources for older adults. May provide resources for adult day care, congregate meals, home delivered meals, homemaking assistance, legal assistance, transportation assistance, personal care, and more.

Provides information and referral about needed social services and will provide outreach services in the home for seniors and those who are shut-ins. Offers assistance with completing various forms including help with simple wills, living wills, Power of Attorney for Health Care, etc.

On site the Center offers Blood Pressure Checks, legal counseling, and a benefit bank. Offers information and referral to governmental and community resources.

Provides a directory of services for older adults called "Resource Guide for Older Adults and their Families". Information in the guide includes how to resolve consumer issues, applying for Social Security, Medicare and Medicaid, contacting mental health and crisis services, housing options for older adults, engaging legal and financial services, planning for long-term care, lifelong learning programs and more. Can also provide individual brochures and information on organizations that deal with specific aspects and health conditions of aging.

Helps disabled older adults remain safe and independent at home. Prevents unnecessary nursing home placements. Consumer-Directed option gives consumers the option to serve as employer by hiring eligible individuals of their choice to provide their personal care services.

Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Refers individuals with aging-related or disability-related issues to appropriate available services in the community to ensure that individuals' needs are fully met. Helps individuals find help for housing, transportation, in-home care, long-term care, and other health and wellness services including benefits screening.
