Pre-Admission Screening System Providing Options And Resources Today (PASSPORT) | Catholic Social Services Of The Miami Valley

Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Senior Center - Recreation / Personal Enrichment | Bay Village City Of - Department Of Community Services

Provides a variety of recreational activities and personal enrichment programs for seniors. Activities and programs include, but are not limited to, exercise classes, line dancing, book clubs, pinochle, canasta, bridge and other games, health and safety presentations, trips, luncheons, gardening, and special events. Offers a group called BV 60+ where members plan group activities and outings. Offers computer tutoring sessions where tutors will answer students questions and teach computer skills such as basic skills, E-mail, and the Internet.

Pro Seniors' Legal Helpline | Pro Seniors

Provides free legal assistance via phone regarding problems that affect older adults living in Ohio. Subject areas include civil law issues such as consumer problems, Medicaid, Medicare, Social Security, subsidized housing, wills and estates. If the legal problem cannot be solved over the phone, the client may be referred to a private attorney who can provide the service. Also offers pamphlets on a variety of legal topics.

Provides a University-wide, interdisciplinary Center that offers numerous continuing education opportunities focusing on aging and health issues for professionals and the public. Helps individuals and their families explore options and develop plans to care for older adults. Program helps to identify a loved one’s strengths, needs, concerns and preferences; to establish goals and implement a plan of care; to connect individuals to community resources and government benefit programs; to coordinate doctor appointments, transportation, and caregiver services. Services include counseling to help cope with issues associated with aging, such as changes in independence, health problems, dementia, death of a loved one, isolation, and family dynamics.
