Program helps to identify a loved one’s strengths, needs, concerns and preferences; to establish goals and implement a plan of care; to connect individuals to community resources and government benefit programs; to coordinate doctor appointments, transportation, and caregiver services. Services include counseling to help cope with issues associated with aging, such as changes in independence, health problems, dementia, death of a loved one, isolation, and family dynamics. Counseling is also available for coping with mental health issues, such as anxiety and depression.
Provides information and referral of social services and benefits information to older adults. Will provide assessments in the home for residents when needed. Offers counseling to help seniors cope with life-style changes, personal, marital, and family problems. Assist with forms completion and maintains a registry of chore service providers

Provides free legal assistance via phone regarding problems that affect older adults living in Ohio. Subject areas include civil law issues such as consumer problems, Medicaid, Medicare, Social Security, subsidized housing, wills and estates. If the legal problem cannot be solved over the phone, the client may be referred to a private attorney who can provide the service. Also offers pamphlets on a variety of legal topics.
