Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Will assist the consumer with gathering any necessary documents and will guide them through applications process (including assistance with filling out forms) as needed. Agency provides follow-up with the client. Goal is to minimize the number of calls and contacts the consumer has to make, and encourage the use of the least costly and least restrictive services.
