Geriatric Services | Ohio District 7 Area Agency On Aging

Provides assessment services that include consultation, evaluation, and planning a course of action for older adults. For enrolled individuals, care managers work with the client and his/her family, physicians, and other professionals (attorneys, etc.). Offers program options, makes recommendations for care, and arranges services.

Geriatric Clinic | Signature Health

Provides outreach to people experiencing mood disorders due to issues related to aging, psychological problems, and other issues affected by the aging process. Services include geriatric assessment, case management, counseling (group and individual), medication monitoring, in-home safety assessments and other services determined by case managers. Services to women include attention to psychiatric issues related to physical stages in their lives.

Resource Guide For Older Adults/Supportive Housing | Western Reserve Area Agency On Aging

Provides a directory of services for older adults called "Resource Guide for Older Adults and their Families". Information in the guide includes how to resolve consumer issues, applying for Social Security, Medicare and Medicaid, contacting mental health and crisis services, housing options for older adults, engaging legal and financial services, planning for long-term care, lifelong learning programs and more. Can also provide individual brochures and information on organizations that deal with specific aspects and health conditions of aging.

University Center On Aging And Health | Case Western Reserve University

Provides a University-wide, interdisciplinary Center that offers numerous continuing education opportunities focusing on aging and health issues for professionals and the public. Programming includes workshops, conferences, and seminars throughout the year. All provide professional CEU's to health and social work practitioners. Also offers a graduate level certificate in Gerontology. The Center also participates collaboratively in Research efforts about Aging and Health with other University Departments/Schools. Also offers a certificate program in Gerontology.

Passport Home Care | Western Reserve Area Agency On Aging

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.
