PASSPORT HOME CARE | WESTERN RESERVE AREA AGENCY ON AGING

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Application process

Phone intake or application, M-F: 8:00am-5:00pm. In addition, social workers or others working on behalf of clients may phone to obtain an intake form. The social worker or other agent must complete the form for the client and return it via fax.

Fee

Free assessment. Most individuals receive services for free, but some clients are required to pay a monthly co-pay.

Eligibility

Must live in Cuyahoga, Geauga, Lake, Lorain, or Medina County. Older adults age 60+ who have income that would make them eligible for Medicaid Long Term Care (income and asset limits), live in a home or apartment and need assistance with daily activities (for example, bathing, dressing, meals) to remain at home. Provisions apply to prevent the spouse of the person needing care from counting the family home as an asset. The package of services must not exceed a certain percentage of the costs of Medicaid nursing home care.
