Application for Housing Assistance can be made electronically or are available for printing on their website. Please apply online or print it and complete it in your own handwriting. Make sure all questions and sections are completed and anything that does not apply to you is marked “N/A”. Incomplete applications will not placed on the waiting list. The application can be completed on-line via the PMHA website. Applicants should make sure their address is correct on the online pre-application and if it isn't, contact PMHA to correct it. The application is in PDF format. You must have Adobe Acrobat installed on your computer in order for the application to print properly. If you do not have Adobe Acrobat, go to the Adobe website to download and install it. This is a free download. Read instructions completely. Connects caregivers with either a Certified Family Peer Support Specialist or a volunteer who has lived experience caring for someone living with a mental, chronic or serious illness. Goal is to change middle and high school students’ knowledge and attitudes toward mental health conditions and toward seeking help.

Provides pouches to be used for the safe disposal of outdated and unwanted medications. The pouches deactivate drugs and render the chemical compounds safe for landfills. Each bag can be used to dispose pills, liquid medication, or medicine patches.

Also provides a mechanism to record hate crimes that have occurred in the service area as well as providing support, information, and referrals to appropriate services for victims of hate crimes. Supports the passage and enforcement of laws and other social measures that protect and promote the rights and interests of gays, lesbians, bisexuals, transgender individuals, cross-dressers, and people who are in the process of coming out or who are questioning their sexual orientation or gender identity.

Assists consumers in evaluating the comparative safety of products and conducts education programs to increase consumer awareness. Maintains free hotline to provide information about recalls and to receive reports on unsafe products and product-related injuries. SNAP-Ed facilitates voluntary adoption of healthy food and physical activity choices and other good nutrition-related behaviors. • Lied to or misled you or any client in the provision of services or billing.
• Suffered discriminatory or culturally inappropriate treatment.
• Treated you or any client while under the influence of alcohol or drugs.
• Released information without your consent.
• Been negligent in your treatment or in the treatment of any client.
• Shown an inability to practice safely and competently for any reason.
