Make sure all questions and sections are completed and anything that does not apply to you is marked “N/A”. Incomplete applications will not placed on the waiting list. The application can be completed on-line via the PMHA website. Applicants should make sure their address is correct on the online pre-application and if it isn't, contact PMHA to correct it.
The application is in PDF format. You must have Adobe Acrobat installed on your computer in order for the application to print properly. If you do not have Adobe Acrobat, go to the Adobe website to download and install it. This is a free download. Read instructions completely.
