Provides case management for individuals living with HIV, including individual support maintaining medical care and medication adherence, assistance with the cost of HIV medical care, medications, dental services, mental health care and health insurance premiums (income guidelines apply), referrals for substance abuse and/or mental health recovery, and help connecting with other service providers and resources.

Manages a drop-in center for teens and young adults, regardless of gender expression or sexual orientation, who would like a safe space to socialize with other youth. Helps those who are able to live independently in the community to locate safe and affordable housing. Offers a directory of available housing facilities, public and private. Emergency financial assistance may be available to assist with meeting rent and/or utility payments and security deposits.

Provides pre-exposure prophylaxis (PrEP) to patients at risk for HIV to prevent getting HIV from sex or injection drug use. Also provides post-exposure prophylaxis (PEP) for individuals who have possibly been exposed to HIV. It is only for emergency situations. PEP must be started within 72 hours after a possible exposure to HIV

Offers anonymous HIV testing, screening, and pre and post-test counseling. Provides medical treatment, information and referrals to community resources, and mental health services to persons infected with HIV who are not yet showing symptoms of AIDS. Can work with individuals to secure medications through patient assistance or insurance programs. Provides case management services to HIV patients. Also offers HIV prevention education for groups.

Provides housing and clinical case management for people living with HIV/AIDS

Federally funded program that helps meet housing needs of individuals with HIV/AIDS. This housing program administers vouchers and other housing assitance for persons (or households with a person) who has AIDS/HIV. This program may have a wait list.

Provides a resource library for relevant information regarding HIV and STI’s. All resources are free to distribute to the public.

Our medical providers offer Pre-Exposure Prophylaxis (PrEP) services to patients to reduce the likelihood of contracting HIV. Our providers are also trained in providing Post Exposure Prophylaxis (PEP) an emergency-based treatment for the prevention of HIV after possible exposure has occurred. Legal services are provided during legal clinics. Referrals to community attorneys who offer pro bono or discounted services are also given.

Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Also provides homemaker assistance (meal preparation, laundry and general household management including light housekeeping.) Services also include personal care and homemaking for adults with HIV/AIDS.

Provides publications and public health releases on AIDS clinical trials.

Provides daily hot meals to persons who are unable to provide meals for themselves. Clients may choose from various menu options including home-style entree of the day (regular and diet), deli cold lunch (regular and diet), vegetarian entrees, mechanical soft, pureed, and kosher entrees. Weekly delivery of frozen meals is available for those who may have frequent medical appointments and treatments.

Provides referrals to specialized medical care, legal guidance, support groups, and other supportive services for individuals living with HIV.

Provides information about the symptoms, preventative measures, screening/diagnostic procedures and methods of treatment and management for HIV.

Offers a variety of support groups for people living with HIV.

Provides medications for the treatment of HIV/AIDS and in some cases, help with applying for other medication assistance programs. Medications are ordered through a mail order pharmacy. This program should be used as a last resort.

Offers limited financial assistance for people living with HIV for utility bill payment assistance, mortgage payment assistance, and rent payment assistance.

Provides one-on-one nutrition and education counseling to help clients take control of their nutritional well-being and lesson the harsh side effects of medications, help the body's immune system fight opportunistic infections, prevent and stop malnutrition.

Provides a variety of food services for those with HIV/AIDS. Provides two grocery bags of non-perishable food, food supplements, special fortified frozen meal delivery (5-day supply delivered each week), and 7 day home-delivered maintenance meals for the homebound (1 hot/1 cold -diabetic and low-sodium accommodated). Offers transportation for medical HIV-related appointments and nutrition counseling.
