Hiv/Aids Health Navigation/ Medical Case Management | Equitas Health

Provides anonymous and confidential HIV testing. Provides comprehensive case management to people living with HIV/AIDS. Services include emergency financial assistance, referrals for medical, dental, legal, and other community resources, counseling, support groups, activities and rental and utility assistance. Outreach through Ohio AIDS/STI Hotline.

Prep/Pep | Equitas Health

Our medical providers offer Pre-Exposure Prophylaxis (PrEP) services to patients to reduce the likelihood of contracting HIV. Our providers are also trained in providing Post Exposure Prophylaxis (PEP) an emergency-based treatment for the prevention of HIV after possible exposure has occurred. Home Support Services | Cuyahoga County Division Of Senior And Adult Services

Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Also provides homemaker assistance (meal preparation, laundry and general household management including light housekeeping.) Services also include personal care and homemaking for adults with HIV/AIDS.

Legal Information Services For Aids/hiv And Lgbtq Issues | Lambda Legal

Provides information and resources regarding discrimination related to sexual orientation, gender identity and expression, and HIV status. Selects cases that will have the greatest impact in protecting and advancing the rights of LGBTQ people and those with HIV. While Lambda Legal is not able to take every case, the Help Desk can discuss legal issues and can provide useful information. This assistance may include follow-up discussions with Lambda Legal attorneys or contact information for an attorney or for other organizations that may directly assist.

Meals On Wheels | Lifecare Alliance

Provides daily hot meals to persons who are unable to provide meals for themselves. Clients may choose from various menu options including home-style entree of the day (regular and diet), deli cold lunch (regular and diet), vegetarian entrees, mechanical soft, pureed, and kosher entrees. Weekly delivery of frozen meals is available for those who may have frequent medical appointments and treatments.
