Residents of Ohio with AIDS/HIV with annual income at or below 300% of poverty. Must have current primary health insurance coverage (includes private insurance, COBRA, and Medicare Part D) with prescription drug coverage that is better or as good as the coverage the program can provide.
Proof of income: Pay stubs for 30 consecutive days Social Security Income award letters (if applicable) IRS Income Tax Transcript Copies of unemployment compensation stubs (if applicable) Physician's verification of HIV infection and lab results (viral load and CD4) no older than six months from date of application.
