40-unit, two story apartment complex. Provide permanent and supportive housing project for individuals with mental health and/or substance abuse issues.

Offers service coordination, advocacy, and public health nurse home visits to assist families in obtaining and coordinating services.

Offers information about local resources and provides assistance with a variety of veteran needs. Responds to requests from veterans and families of veterans seeking financial assistance, veterans benefits information, caregiver resources, etc.

Provides assistance with Medicare consultation and insurance information, agency referrals, home visits for Home Delivered Meal client assessment, educational and health-related programs, non-medical home care agency listings, community resource lists and caregiver consultation and education.

The Disability Information and Access Line (DIAL) helps people with disabilities get connected to information about local community resources that support independent living. DIAL provides information about essential services such as transportation, housing support, disability rights, and more. DIAL is a public service of the U.S. Administration for Community Living (ACL) and is administered by USAging.

Provides referrals to local food pantries for a 3-5 day supply of emergency food.

Provides information on which schools are eligible for the school voucher program. Does not provide applications, information on application process or information on the school voucher program.

Provides educational material and information about mental illness, the mental health care system, and community resources. Referrals are made to treatment providers as appropriate. Also includes a newsletter keeping members and others up-to-date on mental health issues and advocacy.

Provides information about various pregnancy and family planning related issues, and referrals for professional and community services.

Provides information, resources, referrals and education for individuals struggling with gender identity.

Provide Pack N Play portable cribs and Safe Sleep Survival Kits to families and caregivers that are unable to provide a safe sleep environment due to financial hardship. Survival Kit contains a sheet for the Pack N Play, a sleep sack for the infant, a bedtime storybook and educational materials for the parents. Family must come to Board of Health office to pick up Pack N Play and receive instruction on proper assembly and use before taking Pack N Play home.

Provides information and makes referrals to local support agencies to families of developmentally disabled individuals.

Refers individuals with aging and/or disability related issues to appropriate available services in the community to ensure that individuals' needs are fully met. Helps individuals find help for long-term care services including institutional care and community based services. Works in collaboration with partner organizations which include Benjamin Rose Institute, McGregor PACE, County Departments of Job and Family Services, Easter Seals, Linking Employment Abilities and Potential (LEAP), Services for Independent Living, and Western Reserve Area Agency on Aging as well as other organizations within the Aging & Disability Resource Network (ADRN).

Provides services for seniors or permanently disabled adults to remain independent. Services include home visits, help with forms and paperwork, utilities and more.

Provides information on VA benefits; completes VA forms, contacts other agencies that assist veterans.

Provides information and referral on services that would benefit service members (from all branches of the military) and their families.

Provides advocates who will accompany victims to the courts. Advocates can assist in filing for state Crime Victims Compensation Funds, can provide information about local criminal justice procedures, and may refer to local counselors, civil attorneys and clergy. Offers 24-hour line where callers may talk about their situation.

Offers a free, anonymous telephone service that provides telephone counseling, information and referral to individuals who feel overwhelmed, depressed, out-of-control or in need of support. Families and individuals receive support, education, counseling or referrals for their particular issues and questions.
