Provides comprehensive in-home, health-related services to 60+ individuals who meet an Intermediate Level of care. Provides Medicaid and state funds to assist and enable an individual with long-term care needs to remain in their own home. Services paid for by PASSPORT and provided by agencies that PASSPORT contracts with. Some of the services available under this program are: personal care, homemaker, adult day care, transportation, medical supplies, home-delivered meals, and nutrition counseling.

Services include: - Provide Ombudsman Core Services (information, advocacy, and complaint investigation) for dual-eligible members of MyCare Ohio and the community-well. - Provide information regarding rights and benefits to members, families, and providers within the MyCare Ohio service delivery system. - Investigate and resolve complaints regarding MyCare Ohio enrollees and other LTC consumers receiving care from Managed Care Organizations. - Assist and represent MyCare members and families in administrative hearings and appeal situations in accordance with Ombudsman policy.

The Ohio Home Care Waiver Program (OHC) meets the needs of people who prefer to get long-term care services and supports in their home or community, rather than in an institutional setting. CareSource has contracted with the Ohio Department of Medicaid in the role of a Case Management Agency. Medicaid (medical assistance) provides health care coverage to families who may not otherwise have access to health care. The program is designed to safeguard the health and well-being of Highland County residents, particularly children, pregnant women, elderly, and individuals with disabilities. Some adults with children may also be eligible. By keeping the community in good health, Medicaid allows individuals to remain productive members of society. Like health care coverage, Medicaid may pay for part of the cost of visits to the doctor, hospital expenses, nursing home care and home health care. In some cases, transportation to the doctor may also be available. Medicaid programs include Healthy Start; Healthy Families; Healthchek; Pregnancy-related services; and Aged, Blind and Disabled among others. Individuals who receive Medicare, but whose income is too high to qualify for Medicaid, may be eligible for the Medicare Premium Assistance Program.
