Medicaid - Specialized Recovery Services (SEVERE MENTAL ILLNESS / CHRONIC CONDITIONS) | Cuyahoga County Job And Family Services

Provides public insurance to cover medical services that may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Most clients will have to choose and enroll in (will have their care provided by) a Managed Care Plan (MCP). When it is time to renew, Enrollees will receive a packet, including information regarding Medicaid status and forms for re-certifying coverage.

Application process

Individuals who are not already on Medicaid must be referred by their health provider. Those already on Medicaid can update their case information by calling the Eligibility Contact Center: M-F: 8:00am-1:00pm (due to high call volume, caller may be given the option to receive a call back - if they choose this option they should get a call within one business day) or through the website anytime. Most interviews (if needed) can be done by phone. Agency can look up most supporting documents through an electronic records search - the client will be notified if they need to supply any extra supporting documents. Those with general questions about Medicaid, Medicaid Managed Care Plans, or who simply need to change their address/contact information can call the Medicaid Customer Service Hotline M-F: 7:00am-8:00pm; Sa: 8:00am-5:00pm. Starting March 28th, 2025: Clients using the online benefits portal will need to either create or sign into an OHID account. After they have created their OHID and logged into it, they will be asked to set up Multi-Factor Verification (a security feature to protect against identification theft). Those who need help navigating the set up process can phone the Contact Center M-F: 8:00am-4:00pm.

Eligibility

Individuals with a severe mental illness or chronic condition who live in Cuyahoga County and are U.S. citizens. Individuals who are not already on Medicaid must be referred by their mental health provider. Those already on medicaid should contact their caseworkers. Financial eligibility is an income at 225% Federal Poverty Level (FPL) and assets up to $2,000 for an individual, $3,000 for a couple. Some individuals may also be subject to asset limitations. Must have been diagnosed with a severe mental illness, or have been diagnosed with one of the following chronic conditions: such as Cancer, HIV/AIDS or Immune Deficiencies, End Stage Renal Disease, Sickle Cell Anemia, Cystic Fibrosis, Hemophilia. Also serves those who have had a previous transplant as well as those who are active on the solid organ or soft tissue transplant waiting list. Those with chronic conditions must also be over age 21, receive Social Security disability benefits, have one or more inpatient admissions due to the chronic condition, receive a qualifying score on the Adult Needs and Strengths assessment, and reside in an approved Home and Community Based Services setting.
