Offers a heating bill credit to eligible households through the Home Energy Assistance Program (HEAP) one time per year. Assistance is applied directly to the bill for a household's primary heating source. Applications are accepted between July 1st and May 31st.

Civil Rights organization offering discrimination and advocacy assistance, especially in the issues of education, environmental and climate justice, economic opportunity, health, media diversity, criminal justice, and civic engagement.

Accepts applications and determines eligibility for the Social Security Disability Insurance (SSDI) program, which provides monthly payments to people who have a disability that stops or limits their ability to work.

Provides food boxes of non-perishable and select dairy items to individuals 60 and older. Sponsored by the Greater Cleveland Food Bank.

Provides temporary emergency shelter for individuals and their children who have experienced domestic violence/abuse. Case managers work with clients to assist with determining needs while in the shelter as well as developing goals and providing referrals and assistance for after discharge. Staff members inform clients about housing, education and employment opportunities. Medical services and mental issues, treatment and resource needs are also addressed.

Maintains a database of agencies providing services to children. Offers information and referral services to families with children. Referral services include foster care, adoption, counseling services, specialized education and programs for developing disabled youth and youths involved with juvenile justice.

Provides a 30 day disconnection delay for residential customers of PUCO-regulated electric, natural gas, and water utilities who are facing a disconnection for nonpayment in cases where disconnection would be hazardous to the health and safety of a permanent resident of the home. **Form must be completed by a licensed physician, physician assistant, clinical nurse specialist, certified nurse practitioner, certified nurse-midwife, or local board of health physician. *The form may be used no more than three times in a rolling 12-month period.

Offers case management by a certified community health workers. Works to improve birth outcomes, eliminate disparities in infant mortality, reduce low birth weight and preterm births while addressing the social determinants that affect maternal and child health in this population. Utilizes a model that uses 20 Pathways that addresses the social determinants of health to improve health outcomes. Connects clients with needed resources to reduce social deterrents such as transportation and employment, prenatal care for mothers, and safe sleep environments for babies. Services include:

• Provide information on pregnancy and newborn care.
• Offer problem-solving assistance to expectant mothers to resolve issues that may arise during the pregnancy.
• Encourages clients to set and reach health and personal goals.
• Acts as a liaison between the client and other community agencies to connect with resources and services such as medical care, housing, food, transportation, Medicaid, WIC, and other benefits.

Provides zoning services for the city

Offers a food pantry for low-income households in need. Clients may visit once every 30 days.

Provides assistance for people who want to file a petition to legally change their name.

Issues Social Security cards for newborn U.S. citizens and provides replacements for those who have lost their cards. Also assists with name changes on Social Security cards.

Member organization for veterans who are paralyzed due to spinal cord injury or disease. Offers human services, adaptive sports, information, referral and advocacy regarding benefits, legal rights, vocational rehabilitation and other related topics.

Provides 16-week (self-paced) employment training program for persons who are blind or severely visually impaired. Program involves management training, food service safety, sanitation, merchandising, customer service, business math, accounting, restaurant law, taxation and vending machine maintenance/repair, as well as on-site work experience. Service includes exam and licensure. Locations are awarded according to agency selection process.

Provides 16-week (self-paced) employment training program for persons who are blind or severely visually impaired. Program involves management training, food service safety, sanitation, merchandising, customer service, business math, accounting, restaurant law, taxation and vending machine maintenance/repair, as well as on-site work experience. Service includes exam and licensure. Locations are awarded according to agency selection process.

Provides time-limited cash benefits and supportive services to low-income families. Supportive services include a transportation allowance and job search and placement assistance. Recipients are required to participate in a combination of activities and/or work/training programs to remain in the program. These work activities may include community work experience, job search, training, education, and other activities that will assist the applicant with overcoming barriers to employment. Many families receiving cash assistance may also be eligible to receive food assistance, subsidized childcare assistance, and Medicaid health insurance. Once participants become employed and are no longer eligible for cash benefits, they may continue to receive Medicaid and child care benefits for up to one year or until their income exceeds allowable limit. In addition, clients who transition to work may still be eligible for food assistance (depending on income).

Provides 24 hour support to address the needs of victims of violent crime (such as rape, incest, domestic violence, assault, stalking, drunk driving, hate crimes, and others). Trained advocates provide information and referrals, support, and education. Works to treat survivors fairly and with respect when accessing all services.
