Our neighborhood navigators help connect pregnant and postpartum women to valuable community resources to support them during pregnancy and up to one year after birth. Our goal is to help support a healthy birth outcome and ensure that infants make it to their first birthday. Our navigators help pregnant people connect to prenatal care and support through home visiting programs, referrals for mental health services, and access to support for smoking cessation and other substance misuse programs. Additionally, our navigators make sure expecting and postpartum mothers have everything they need to welcome their baby home by providing access to baby items, cribs, and other material goods to meet their needs. We also provide referrals to non-baby related resources, as we recognize that the health and well being of the mother can directly impact the health of the child. We can serve any pregnant mother living in Montgomery County, or any mother with an infant under one year of age. —
