Provides health care coverage consisting of a primary and acute care benefit package and long-term care if a person has the required level of care need. Senior And Adult Services Gatekeeper Program | Summit County Public Health

Proactively works with local agencies to identify older people 60 years of age and over and adults with known or suspected Alzheimer's disease or a related dementia, who are living alone, have an intellectual developmental disability and/or have caregivers in needs of support. Once a referral is received a nurse and social worker will make a home visit to assess the situation, begin linking to community resources and provide case management support.
