Senior And Adult Services Gatekeeper Program | Summit County Public Health

Proactively works with local agencies to identify older people 60 years of age and over and adults with known or suspected Alzheimer's disease or a related dementia, who are living alone, have an intellectual developmental disability and/or have caregivers in needs of support. Once a referral is received a nurse and social worker will make a home visit to assess the situation, begin linking to community resources and provide case management support.

Finds ways to meet members' needs through organized social and educational events, volunteer assistance, referral to service providers, and opportunities for engagement within the Clintonville-Beechwold Neighborhood. This program includes weekly social opportunities, with outings suggested by the members. This program also includes a wide range of volunteer services including transportaiton to medical appointments, the grocery store or social outings, snow removal, technology assistance, and more.

Eligible work may include, but not be limited to: electrical (replace fuse or light bulb, repair/replace a switch, repair/replace frayed cords/plugs, etc.); plumbing (repair/replace toilets and sinks, leaking faucets, drain and supply lines, vent pipes and sump pumps, etc.); masonry (repair sidewalks, porch floor and steps; apply waterproofing compound to basement walls; minor tuck-pointing, etc.); carpentry/maintenance (repair doors, jambs, frames, hinges, window frames, kitchen cabinets, decking, floors, porches, holes in walls; install glass; install smoke detectors; install deadbolt locks; etc.); and heating and air conditioning (repair/replace furnace and air conditioner).

Provides nonemergency door-to-door transportation within Lorain County for older adults who require routine medical services and who are unable to use other available means of transportation. Provides door through door assistance and can accommodate wheelchairs and other mobility assistance devices.
