Provides legal advice or representation to handicapped or elderly persons who have problems related to income, employment, family matters, health care, or individual rights.

Offers privately owned rental housing that is made available to low-income households at reduced rates based on a contract between HUD or the state housing authority and the property owner.

Administers home and community-based supports for older adults and those with disabilities. Programs include but are not limited to: PASSPORT, Assisted living Medicaid waivers, long term care consultations, caregiver support services, regional long term care ombudsman, home repair, healthy lifestyles and wellness programs, aging and disability resource center, Medicare Part D enrollment assistance. Additional services may include: legal assistance, case management, transportation, personal care, housekeeping, adult day care, home delivered meals, emergency response systems, and other community resources.

Provides financial assistance for minor home repair projects to low-income seniors and individuals who are permanently physically disabled. Eligible work may include, but not be limited to: electrical (replace fuse or light bulb, repair/replace a switch, repair/replace frayed cords/plugs, etc.); plumbing (repair/replace toilets and sinks, leaking faucets, drain and supply lines, vent pipes and sump pumps, etc.); masonry (repair sidewalks, porch floor and steps; apply waterproofing compound to basement walls; minor tuck-pointing, etc.); carpentry/maintenance (repair doors, jambs, frames, hinges, window frames, kitchen cabinets, decking, floors, porches, holes in walls; install glass; install smoke detectors; install deadbolt locks; etc.); and heating and air conditioning (repair/replace furnace and air conditioner).

Provides mutual support groups for caregivers of older adults, a library with videos and reading materials on caregiver issues, and hosts meetings where doctors, therapists, counselors, facility administrators, and agency staff present information helpful to those caring for older adults. Includes an evening meal.

Offers general health evaluations and examinations. Services included screening tests, nutrition education, counseling and health and fitness education.

Offers a tax reduction program which provides Medina County homeowners with relief on a portion of their home value. With homestead, eligible homeowners receive an exemption on a portion of appraised value from taxation for a single family home. Eligible military Veterans receive an additional exemption. Will answer questions and assist with completing forms regarding the reduction.

Links older adults with appropriate community support services that can help prevent premature admission to institutionalized care.

Transportation provided by the Champaign Transit System.To and from community resources for the purpose of applying for receiving services, reducing isolation and promoting independent living.

Presenting a safety health fair for older adults with senior service provider booths, crime safety awareness presentation and senior self-defense class. Provides a free lunch to participants. Offered through a partnership with Summit County Sheriff.

Provides information for older adults and their caregivers who are seeking assistance in long-term care decisions and options. In-home consultants are available to identify an individual's care needs, and discuss service options and program eligibility.

Offers geriatric assessments to adults 70 and older
