The Eldercare Locator database includes State Agencies on Aging, Area Agencies on Aging, Title VI Native American Aging Programs, Aging and Disability Resources Centers, and aging information and referral programs.

Information and referral for services assisting older adults, including programs available through national, state and local resources.

Proactively works with local agencies to identify older people 60 years of age and over and adults with known or suspected Alzheimer's disease or a related dementia, who are living alone, have an intellectual developmental disability and/or have caregivers in needs of support. Once a referral is received a nurse and social worker will make a home visit to assess the situation, begin linking to community resources and provide case management support.

Central Community House (CCH) will offer a variety of technology-oriented classes, workshops, tech cafes and one-on-one assistance for seniors, focusing on topics such as digital safety, managing administrative tasks online, utilizing social media, creating shared projects and using technology for daily tasks (apps for fitness, medical reminders, banking, etc.). Participating seniors will also receive a free device to ensure they have the means to put into practice what they have learned through the Technology Access Project (TAP).

Provides certified counseling to help with Medicare issues and drug plan open enrollment. Helps older adults access services, benefits, and other programs. Provides assessment of needs and makes referrals to appropriate agencies. Advocates to resolve problems older adults may encounter with governmental and social service agencies.

Provides in-home assistance with personal care, homemaking, emergency response systems, meals on wheels as alternative to nursing home placement. Services for Older Adults includes Life Essentials, which has guardianship and intervention for seniors with mental illness.

Offers a tax reduction program which provides Jefferson County homeowners with relief on a portion of their home value. With homestead, eligible homeowners receive an exemption on a portion of appraised value from taxation for a single family home. Eligible military Veterans receive an additional exemption. Will answer questions and assist with completing forms regarding the reduction.

Senior Program Director or Social Service Director will provide information and referral on topics specific to the 60 and over population, including Social Security, Medicare, HEAP, Golden Buckeye Cards, applications for government assistance and other services.

Provides monthly deliveries of shelf stable, fresh, and frozen foods for older adults, disabled adults, and individuals in recovery that are living in subsidized housing units in Medina County.

Refers individuals with aging and/or disability related issues to appropriate available services in the community to ensure that individuals' needs are fully met. Works in collaboration with partner organizations which include Benjamin Rose Institute, McGregor PACE, County Departments of Job and Family Services, Easter Seals, Linking Employment Abilities and Potential (LEAP), Services for Independent Living, and Western Reserve Area Agency on Aging as well as other organizations within the Aging & Disability Resource Network (ADRN).
