Provides health care coverage consisting of a primary and acute care benefit package and long-term care if a person has the required level of care need. Covered services may include prescription drugs, home care, doctor visits, hospital care, laboratory and x-rays, medical equipment and supplies, dental care, transportation, mental health, vision services, long-term care, alcohol and drug rehabilitation and other services.

Grief support group for adults. Facilitated by masters level social worker. Held in the library area of the Jewish Community Center.

Provided at 4 dining room sites around Clark County for individuals who benefit from a nutritious daily meal in a social setting. Locations include: United Senior Services at 125 W. Main, Northhill Towers, 1416 Lake Ave, New Carlisle, and Tubman Towers.

Provides mutual support groups for caregivers of older adults, a library with videos and reading materials on caregiver issues, and hosts meetings where doctors, therapists, counselors, facility administrators, and agency staff present information helpful to those caring for older adults. Includes an evening meal.

Provides a discount card for Ohio's older adults and permanently disabled residents. Card holders are able to receive discounts on various services at participating local businesses. For a list of participating businesses, visit website. Also includes the Ohio's Best Rx prescription drug program.

Provides between one and 12 vouchers per month which can be used at the Soprema Cafe. The vouchers can be exchanged for either a hot meal or a sandwich. Fruit, vegetables and milk are also part of the restaurant voucher.

Provides informational guides about financial decisions that affect retirement, whether early stages of retirement planning or looking for ways to grow retirement savings.

The Eldercare Locator website contains a searchable database to help users find a range of resources at the state and community levels. The database includes State Agencies on Aging, Area Agencies on Aging, Title VI Native American Aging Programs, Aging and Disability Resources Centers, and aging information and referral programs.

Provides transportation for older adults to doctors' appointments, drug stores for prescriptions, county-wide agencies for required appointments, as well as for grocery shopping and other essential local travel needs. Staff will also shop for groceries for clients.

Proactively works with local agencies to identify older people 60 years of age and over and adults with known or suspected Alzheimer's disease or a related dementia, who are living alone, have an intellectual developmental disability and/or have caregivers in needs of support. Once a referral is received a nurse and social worker will make a home visit to assess the situation, begin linking to community resources and provide case management support.

One and two dedroom units. Trash, water, sewer, electric are included in the rent. Small pets (20 lbs or less) with a pet deposit and service pets permitted.

Provides older adults $50 worth of coupons that can be used at participating farmers' markets and roadside stands for fresh and locally-grown produce, herbs and honey from June-October or can select a pre-packed produce box ($50 value).

Provides 1-bedroom suites, appliances, individual air conditioning and heating, a craft room, on site laundry, and other amenities. All utilities are included except telephone and cable.

Provides help to older adults who want to remain independent and living in the community. Enrollees receive personalized services that are coordinated by a care manager. The care manager can assist with things like disease management, health insurance benefits, service coordination and transitional care. Services may include adult day care, chore, homemaker, home delivered meals, personal care, safety monitoring and minor home repair.

Program designed for adults with severe and persistent mental illness to learn basic life skills as well as further their education and employment goals.

Provides home repair and rehabilitation to improve safety and lower health risks. Note: this is not an emergency program.
