Care manager assists older adults and/or caregivers with coordinating and accessing services, which may include the following: * Alzheimer's Respite Program * Family Caregiver Support Program * Adult day care * Education, information, and assistance * Homemaker services * Personal care services * Safety monitoring * Meals * Minor home modifications

Offers free van transportation service for non-emergency medical appointments (except following a surgical procedure) and errands for older adults and adults with disabilities.

Offers social activities and services for seniors, incuding bingo, a meal on Tue and Thu, and exercise classes. Provides Golden Buckeye Card applications. Available for rentals on weekends and evenings.

Offers recreational and social activities (Golden Agers group) and other services older adults. Provides blood pressure screenings, cholesterol tests, BINGO, line dancing, an open art studio and a congregate lunch for seniors.

C.A.R.E. offers assessments and evaluation by USS trained staff for services/programs at United Senior Services and referrals to agencies and support programs in the community.

Provides mutual support groups for caregivers of older adults, a library with videos and reading materials on caregiver issues, and hosts meetings where doctors, therapists, counselors, facility administrators, and agency staff present information helpful to those caring for older adults. Includes an evening meal.

Provides case management to qualified individuals. Provides a detailed assessment to determine level of care needs and explanation of available services and ongoing supportive services such as monitoring and service coordination. Assistance with guardianship referral or nursing home placement available if needed.

Provides system of care for Franklin County older adults to assist them in remaining independent. Provides information, advocacy and case management. Will link older adults to resources such as homemaker services, personal care, caregiver relief, emergency response systems, home delivered meals, medical transportation, adult day services and minor home repair.

Provides continuing care retirement community consisting of two apartment high rises,75 condominium-style housing units and 154 Brownstone units in a supportive independent living environment.

Offers in-house and outreach therapy counseling, case management and advocacy for older adults and their families. Provides assistance with medical and/or insurance forms. Transportation available with prior arrangement.
