Assists individuals with mental illness with accessing needed supports and services in order to enhance daily life skills. The goal is to improve independence in the community and reduce the impact of mental illness on daily functioning.

Case management, referrals and advocacy for seniors needing to access community services in order to live independently in their home. Some limited chore assistance is available.

Recruits and coordinates the placement of volunteers who are 55 or older into various organizations around Ohio. Includes health care facilities, schools, community and youth centers, non-profit agencies, and government agencies. * RSVP is sponsored by the Corporation for Ohio Appalachian Development.

Serves as clearinghouse for information, resources, and assistance for older adults who live in Greene County and their families. Also provides support to help keep seniors in their own or family member's home, which is arranged through care managers.

Program designed for adults with severe and persistent mental illness to learn basic life skills as well as further their education and employment goals.

Arranges for in-home respite or adult day services to enable caregivers to be temporarily relieved from their care giving responsibilities.

Prepares and delivers hot meals to people age 60 and over who are unable to provide adequate meals for themselves.

Refers individuals with aging and/or disability related issues to appropriate available services in the community to ensure that individuals' needs are fully met. Helps individuals find help for housing, transportation, in-home care, long-term care and other health and wellness services. Offers an internet-based, counselor assisted program that helps older adults and individuals with disabilities file for public benefits. Includes Healthy Start and Healthy Families, Home Energy Assistance Program (HEAP), child care subsidies, food stamps, voter registration and more. Provides benefit navigation, assistance with Social Security and Medicaid applications, and Social Security benefit analysis.

Provides subsidized housing for seniors (HUD). One bedroom units only. Two individuals maximum occupancy per unit. Pet friendly.

Provides counseling, substance abuse and prescription abuse treatment and nurse outreach to older adults and their families.

Information and referral for services assisting older adults, including programs available through national, state, and local resources.

Provides nonemergency door-to-door transportation within Lorain County for older adults who require routine medical services and who are unable to use other available means of transportation. Possible destinations include medical, dental, and vision appointments and hospitals or centers for tests or other regular treatment. Provides door through door assistance and can accommodate wheelchairs and other mobility assistance devices.

Affordable housing units for individuals 62 and older. Units are offered at a low rate to allow comfortable, safe living for tenants who are on a fixed income.

Provides needs assessment, information, socialization opportunities, and referrals to community resources for senior citizens.

Provides emergency assistance based on Ohio Department of Job & Family Services income guidelines.

Proactively works with local agencies to identify older people 60 years of age and over and adults with known or suspected Alzheimer's disease or a related dementia, who are living alone, have an intellectual developmental disability and/or have caregivers in needs of support. Once a referral is received a nurse and social worker will make a home visit to assess the situation, begin linking to community resources and provide case management support.

One and two dedroom units. Trash, water, sewer, electric are included in the rent. Small pets (20 lbs or less) with a pet deposit and service pets permitted.

Provides older adults $50 worth of coupons that can be used at participating farmers' markets and roadside stands for fresh and locally-grown produce, herbs and honey from June-October or can select a pre-packed produce box ($50 value).

Offers privately owned rental housing for older adults and individuals with mobility issues that is made available to low-income individuals and families at reduced rates based on a contract between HUD and the property owner. Provides 1-bedroom suites, appliances, individual air conditioning and heating, a craft room, on site laundry, and other amenities. All utilities are included except telephone and cable.
