Provides nonemergency door-to-door transportation within Lorain County for older adults who require routine medical services and who are unable to use other available means of transportation. Possible destinations include medical, dental, and vision appointments and hospitals or centers for tests or other regular treatment. Provides door through door assistance and can accommodate wheelchairs and other mobility assistance devices.

The Eldercare Locator website contains a wealth of information including a searchable database. The database helps users to find a range of resources at the state and community levels. The Eldercare Locator database includes State Agencies on Aging, Area Agencies on Aging, Title VI Native American Aging Programs, Aging and Disability Resources Centers, and aging information and referral programs.

Provides advocacy and information to help people, strengthen communities and fights for the issues that matter most to seniors and thier families.

Provides older adults $50 to be used across the state for fresh locally-grown produce and herbs at area farmers markets and roadside stands. Participants have the choice to use a mobile application or a physical card.

One and two bedroom units. Trash, water, sewer, electric are included in the rent. Small pets (20 lbs or less) with a pet deposit and service pets permitted.

Provides short term case management services that include consultation, evaluation and planning a course of action for older adults. Offers options and makes recommendations for care. Works with the client and his or her family members, physicians, and other professionals such as attorneys. Service includes explaining what is available, discussing eligibility requirements and financial resources required, determining needs and wishes of clients, and helping to arrange services.

Provides specialized psychiatry, counseling and case management services to support the needs of older adults residing in assisted living/nursing facilities or at home who may experience barriers accessing care.

Provides home health care, personal care, and homemaking services.

Provides hot, nutritionally balanced meals and an opportunity to socialize with others. Offers meals on-site at some locations, and to-go at others.

Affordable housing units for individuals 62 and older. Units are offered at a low rate to allow comfortable, safe living for tenants who are on a fixed income.

Proactively works with local agencies to identify older people 60 years of age and over and adults with known or suspected Alzheimer's disease or a related dementia, who are living alone, have an intellectual developmental disability and/or have caregivers in needs of support. Once a referral is received a nurse and social worker will make a home visit to assess the situation, begin linking to community resources and provide case management support.

Finds ways to meet members' needs through organized social and educational events, volunteer assistance, referral to service providers, and opportunities for engagement within the Clintonville-Beechwold Neighborhood. This program includes weekly social opportunities, with outings suggested by the members. This program also includes a wide range of volunteer services including transportaiton to medical appointments, the grocery store or social outings, snow removal, technology assistance, and more.

Offers a free to-go meal to older adults residing in Medina County.

Senior Center provides weekly leisure activities such as playing cards, puzzles, dominoes, movies and other social activities.

Offers privately owned rental housing that is made available to low-income older adults and/or individuals with disabilities at a reduced rate.

Provides public insurance to cover medical services that may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Medicaid is the payer of last resort - Medicare will be billed first if client is eligible for it.

Offers privately owned rental housing that is made available to low-income households at reduced rates based on a contract between HUD or the state housing authority and the property owner.

Offers one-bedroom low income subsidized housing for individuals 62 and over.

Offers a reduction related to income on real estate taxes on residences.
