Provides programs administered by the state that provides medical benefits for individuals and families with limited incomes who fit into an eligibility group that is recognized by federal and state law. Programs include the following: - Modified Adjusted Gross Income (MAGI): Medicaid for parents, caretaker relatives, pregnant women, newborns, children, extension adults, transitional Medicaid, and possible dual eligibility with Aged Blind and Disabled Medicaid and Medicare recipients. - Aged Blind and Disabled (ABD): Categorically Needy. Medicaid for 65 and older. Meets Social Security Administration (SSA) definition of blind or disabled. - MPAP: QMB, SLMB, QI-1, QWDI: Medicare Premium Assistance for those in receipt of Medicare. - MBIWD: Medicaid for individuals between ages of 16 and 64 who meet the SSA definition of blind and disabled and are working. Possible premium required. - SRS: Medicaid for those age 21 and over who are receiving SSI or SSDI, meet SRS diagnosis, and are in receipt of another Medicaid category. - Non-Citizen Emergency Medicaid Assistance (NCEMA): Apply at any time. Requires state approval to receive Medicaid eligibility and span. - LTC and Waiver Services: Medicaid for individuals in nursing facilities, assisted living, or home care.

Offers site-based Section 8 / Low Income Housing. Consists of 40 1-bedroom units designated for older adults and/or individuals with disabilities. Two- and 3-bedroom units are available for families.

Provides linkage to social service agencies, home health care services, support groups, telephone reassurance and so forth. The purpose of these services is to be a source of information for older adults and their families and to facilitate older adults with maintaining independence.

Provides hot, nutritionally balanced meals and an opportunity to socialize with others. Offers meals on-site at some locations, and to-go at others.

Offers older adults programming including group exercise classes and aquatic programs that concentrate on mobility and flexibility, dance programs, educational programs, social events, day and overnight trips and volunteer activities.

Offers privately owned rental housing that is made available to older adults and adults with disabilities at a reduced rate.

Provides education on services available for seniors and how to access those services. Also Assists with forms and applications for Medicare, Heating Assistance, or Supplemental Food Programs to help the individual stay in their home.

Offers interior home repairs and accessibility modifications designed to increase senior homeowner's ability to live independently as they age. Projects range from addressing accessibility issues to installing grab bars to replacing lower-threshold showers.

Care manager assists older adults and/or caregivers with coordinating and accessing services, which may include the following: * Alzheimer's Respite Program * Family Caregiver Support Program * Adult day care * Education, information, and assistance * Homemaker services * Personal care services * Safety monitoring * Meals * Minor home modifications

Affordable housing units for individuals 62 and older. Units are offered at a low rate to allow comfortable, safe living for tenants who are on a fixed income.

- Develops a plan for the care of older adults, people with disabilities and people with chronic health issues so the clients can return to or remain in their homes. Also serves juveniles and individuals with developmental disabilities, substance use disorders and mental health issues. - Services include assessing the client's needs, coordinating the delivery of needed services, completion of paperwork required by authorized providers, ensuring services are obtained in accordance with the case plan and monitoring progress to ensure that services are having a beneficial impact on the client.
