Coordinates with other agencies in the community to ensure at-risk-families are receiving as many forms of assistance as possible, such as social, medical and educational services. Community Care Coordinators meet clients in their own homes to assess the family's needs and to establish a care management plan. Targets pregnant women to support throughout their pregnancy to help reduce and eliminate Ohio's Infant Mortality Rate. Offers diapers and wipes to families at all visits by the coordinators.

Help Me Grow Home Visiting is Ohio's program for expectant parents, newborns, infants and toddlers. The program provides health and developmental services so that children start school healthy and ready to learn. Help Me Grow works with parents to support young children's development. Home Visitors provide on-going home visits in the home; focus on improving prenatal health habits and infant care giving skills; provide information on child's health, growth, education and development through the Parents As Teachers program; provide screenings for health, hearing, vision and development; connect families with community resources and services. Affiliated with the evidence-based home visiting model Parents As Teachers. Services include - Developmental Screening - Information on parenting tips - Assistance in developing and reaching family goals - Home Visits to help each child stay "on the road" to healthy growth and development - Service Coordination - professional nurses, teachers, or other qualified staff who share their knowledge about available resources, assist families in connecting with needed services, coordinate services to help families meet their needs and goals. The goals of the Maternal and Child Health (MCH) Division are to eliminate health disparities, improve birth outcomes, and improve the health status of Licking County’s women, infants, children, youth and families. MCH Priorities: - Decreasing childhood obesity, reducing food insecurity, and supporting breastfeeding; - Decreasing infant mortality by ensuring infants have a safe sleep environment and car safety seats; - Increasing parenting skills, promoting early childhood development and school readiness; - Providing referrals to safety net services assure that the health needs of women and children are met; - Addressing social determinants of health; - Free transportation to medical appointments - LCHD has partnered with Uber to help women get a free ride to their next medical appointment. Ride recipients must be 18 and 44 years old and have the Uber app downloaded on their phone. To schedule a ride, call 740-349-6535 and ask for the Uber Health Program.
