Help Me Grow Home Visiting is Ohio's program for expectant parents, newborns, infants and toddlers. The program provides health and developmental services so that children start school healthy and ready to learn. Help Me Grow works with parents to support young children's development. Home Visitors provide on-going home visits in the home; focus on improving prenatal health habits and infant care giving skills; provide information on child's health, growth, education and development through the Parents As Teachers program; provide screenings for health, hearing, vision and development; connect families with community resources and services. Affiliated with the evidence-based home visiting model Parents As Teachers. Services include - Developmental Screening - Information on parenting tips - Assistance in developing and reaching family goals - Home Visits to help each child stay "on the road" to healthy growth and development - Service Coordination - professional nurses, teachers, or other qualified staff who share their knowledge about available resources, assist families in connecting with needed services, coordinate services to help families meet their needs and goals. - Offers pregnancy care coordination services to pregnant high risk women by certified community health workers. - Works to improve birth outcomes, eliminate disparities in infant mortality, reduce low birth weight and preterm births while addressing the social determinants that affect maternal and child health in this population. - Connects clients with needed resources to reduce social deterrents such as transportation and employment, prenatal care for mothers, and safe sleep environments for babies. Services include: - Provide information on pregnancy and newborn care. - Offer problem-solving assistance to expectant mothers to resolve issues that may arise during the pregnancy. - Encourages clients to set and reach health and personal goals. - Acts as a liaison between the client and other community agencies to connect with resources and services such as medical care, housing, food, transportation, Medicaid, WIC, and other benefits.

Provides central intake to women who are pregnant or postpartum to assist in locating resources and coordinate assistance by way of Neighborhood Navigators in the client's preferred setting (home, agency and/or community setting). Performs a detailed screening to identify barriers, needs, and opportunities for women and their families and then a step-by-step approach to action is taken to each item of need. Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid.

Offers free in-home visits by a registered nurse to all new mothers and their babies who deliver at Mount Carmel facilities. The nurses provide wellness checks and assessments, answer questions and address any parental concerns.

Home visits may include: - Supporting preventive health and prenatal practices; - Assisting mothers on how to best breastfeed and care for their babies; - Promoting parents' use of positive parenting techniques; and - Working with parents to set goals for the future, continuing their education, and find employment and childcare solutions. Visits are made to clients in their own homes or other locations as arranged.

Coordinates with other agencies in the community to ensure at-risk-families are receiving as many forms of assistance as possible, such as social, medical and educational services. Community Care Coordinators meet clients in their own homes to assess the family's needs and to establish a care management plan. Targets pregnant women to support throughout their pregnancy to help reduce and eliminate Ohio's Infant Mortality Rate. Offers diapers and wipes to families at all visits by the coordinators.
