Moms And Babies First | Greenleaf Family Center

Offers a prenatal support program for low-income African American pregnant women. Provides in home case management services through Community Health Workers (CHW) who make regular home visits throughout the duration of the pregnancy and until the infant turns one (1) year old. In addition, offers support, education and resources via a telehealth option.

Application process

Referrals to Moms and Babies First can be through the Help Me Grow website at [email protected]. Participants may also call the Moms and Babies First Program directly at 330-376-9494 at extension 232. Participants will speak to the Program Director for Moms and Babies First located at Greenleaf Family Center.

Serves African American pregnant women in their first or second trimester of pregnancy residing in Summit County.

Languages

Works with community agencies for translation services to assist non-English speaking clients. American Sign Language interpreters on staff.
