Provides eligibility counseling and financial pre-screening for State and Federal long-term care services.
Provides person-centered support to older adults and individuals with disabilities as they work through programmatic and eligibility determination processes. Includes: - Exploring the needs expressed by an individual during completion of the Long-Term Services and Supports Questionnaire (LTSSQ). - Identifying potential services and supports that may help meet those needs. - Providing linkage to services and supports identified. - Supporting individuals as they determine which path will best meet their needs. - Determining the level of support individuals may need to take next steps. - Providing appropriate level as of on-going support to individuals based on their needs.
Assists the elderly and their families to explore available living options. A social worker will help research sites, plan visits and assist client at the visits, and then work with the family while client relocates.

Information provided may be on a wide range of long-term care issues such as: assisted living facilities, skilled nursing facilities, hospice care, home health services, respite care, adult day services and insurance issues. If needed, will check the client's eligibility for appropriate government benefits and other community services. Will assist the consumer in gathering any required documents and completing applications. Agency provides follow-up with the client.
