Pro Seniors Home Options Ombudsman staff assist callers with understanding and comparing their long-term care options, including home care, assisted living and nursing homes, in southwest Ohio.

Offers information about nursing homes, assisted living facilities and home care providers. Also includes information about residential treatment facilities for individual living with mental illness.

Assists consumers with selecting a long term care facility by providing information about the characteristics and types of services that are provided by facilities. Does not recommend or rate facilities but can provide the rating that Medicare gives facilities. Helps consumers identify long-term care facilities that match the individual’s needs and choices. Information available includes: verified complaints that were handled by agency. Deficiencies cited by the state department of health.

Provides person-centered support to older adults and individuals with disabilities as they work through programmatic and eligibility determination processes. Includes: - Exploring the needs expressed by an individual during completion of the Long-Term Services and Supports Questionnaire (LTSSQ). - Identifying potential services and supports that may help meet those needs. - Providing linkage to services and supports identified. - Supporting individuals as they determine which path will best meet their needs. - Determining the level of support individuals may need to take next steps. - Providing appropriate level as of on-going support to individuals based on their needs.

Provides assistance to make decisions about long-term care, home health care versus alternate living options as well as projected costs. Offers options counseling regarding in-home and out-of-home care including payment options.
