- Provides short-term assistance with appropriate placement planning and information on long term care services. - Offers information about insurance issues, available benefit programs, and types of facilities and care alternatives, including assisted living, skilled nursing, hospice, home health, respite care, adult day services and other home and community-based care. - Provides eligibility counseling and financial pre-screening for State and Federal programs.

Functions as an Aging and Disability Resources Center (ADRC), offering information and assistance, options counseling and benefits screening for older adults and adults with disabilities.
