Serves Medina County residents age 60 or older and individuals with a disability.

Agency may request personal information or financial documentation to assist with any eligibility or financial pre-screening.

Call the office Monday through Friday from 8:00am to 4:30pm for service or leave voicemail 24 hours a day, 7 days a week.

  • Provides short-term assistance with appropriate placement planning and information on long term care services.
  • Offers information about insurance issues, available benefit programs, and types of facilities and care alternatives, including assisted living, skilled nursing, hospice, home health, respite care, adult day services and other home and community-based care.
  • Provides eligibility counseling and financial pre-screening for State and Federal programs.

Provides in-home and community based services, including home-delivered (Meals-On-Wheels) and congregate meals, and a variety of cultural, recreational and educational programs, to Medina County residents over the age of 60. Functions as an Aging and Disability Resources Center (ADRC), offering information and assistance, options counseling and benefits screening for older adults and adults with disabilities.
