Helps seniors and their family members begin to plan earlier for the potential long-term care needs of loved ones while there is still sufficient time to take a calm and informed look at the situation and all of the options and resources available.

Included are planning for families with a member who requires an increased/increasing level of care; recommendations and referrals to address problem areas; counseling to deal with life change, loss, anxiety and depression; physical, occupational, or speech therapy; skilled home nursing; personal emergency response systems that alert a remote agency when someone has fallen and requires assistance. Service includes caregiver training.
