Eligibility

Adults, age 18 and older, with developmental disabilities and eligible for services from a county board of developmental disabilities, as determined by either the level of care (LOC) or Ohio Eligibility Determination Instrument (OEDI). For Guardianships: must be declared incompetent by the court.

Application process

Any person or organization may make a referral to the agency by submitting a Referral Application. Check the website for information. Or phone the administrative office M-F: 9:00am–5:00pm.

Provides guardianship and other decision-making supports for adults with developmental disabilities who are unable to provide informed consent, represent themselves, or manage their own affairs. Guardianships are for persons declared incompetent by the court and unable to manage their affairs. Will help investigate the need for services for those who feel they may be eligible. Maintains an on-call system for timely responses to emergency medical situations by providing informed consent.
