Bringing Healthy Home is a program of Hocking Athens Perry Community Action and the Southeast Ohio Foodbank that is funded by the Ohio Department of Health .The program provides nutritious food .education, and case management to pregnant and postpartum individuals and their families.
Program includes:
- Food Deliveries - once a month during pregnancy and once a month for 4 months following birth. The amount of food is dependent on family size.
- Home Visits & Case Management - will occur at the time of food delivery.
- Group Educational Activities - there will be several opportunities to attend educational events with the programs cohorts.
- Communication - active communication with program staff is encouraged.
