Investigates and reports findings to entities who have authority to act on information reported by this agency. Issues reports and may recommend changes in county rules if investigations uncover problems system-wide. This is not a law enforcement agency.

The Columbus Aquatics Center is home to six 25-yard swim lanes, an open area for casual swimming, a separate baby pool and a diving well with 1-meter and 3-meter diving boards. The center offers swimming lessons for beginners, intermediate and competition levels as well as lap swim and fitness classes.

Service Associates will interview client about reading preferences and record requests for specific titles, authors and general categories. Delivery is once a month at a prearranged time. All items must be returned at the next delivery unless arrangements are made.

The Trailblazers is a program at Stonewall Columbus for LGBTQ Adults ages 55+ that was created through a generous gift from The Legacy Fund. Trailblazer events include seminars, computer workshops, The Trailblazer Newsletter, and Social Outings. Each year Stonewall hosts a series of seminars on issues ranging from legal issues, healthcare and computer use. The Trailblazer Newsletter goes out via email about twice a month and highlights a variety of upcoming events and outings. Our popular social outings happen at least once a month where Trailblazers meet at the Center on High and get a ride to the theater or a concert, and then finish up at Union Cafe for discounted food and drinks. The Stonewall Columbus Pride Festival has a special Trailblazer Lounge where you can get out of the sun, relax, and even use separate restrooms. The Complex Medical Help Program (CMH) formerly known as the Bureau for Children with Medical Handicaps, is a health care program in the Ohio Department of Health (ODH). CMH links individuals with special health care needs to a network of quality providers and helps individuals and their families obtain payment for needed services. Major components of the program include: conducting quality assurance activities to establish standards of care and to determine unmet needs of children with handicaps and their families; funding services for the diagnosis and treatment of medically eligible conditions; collaborating with public health nurses and local health departments to assist in increasing access to care; supporting service coordination for children with selected diagnoses; and assisting families to access and utilize appropriate sources of payment for services for their child.

Operates approximately 28 institutions confining approximately 49,000 inmates. Three (3) of those institutions house female inmates - the Ohio Reformatory for Women, the Dayton Correctional Institution, and Northeast Reintegration Center. The Franklin Medical Center serves as a medical hospital. The Allen/Oakwood Correctional Institution houses male inmates in need of intensive psychiatric treatment. The remaining institutions house male inmates of varying security levels. The State of Ohio also has two (2) privately-operated prisons: The Lake Erie Correctional Institution and the North Central Correctional Complex in Marion.

Attempts to locate the person abroad when family or friends believe that person may be missing, sick, under arrest, affected by a natural disaster, abuse/neglect (children) or other emergency situations. Will also locate the person in order to deliver an emergency message (such as a death in the family) from their family in the U.S., when the family does not know their whereabouts.

Urgent Care Polaris Parkway provides same-day care for most minor illnesses and injuries. Conditions treated include animal and insect bites and stings, asthma flare-ups (wheezing, not improving with inhalers, no difficulty breathing), cold, flu or COVID-19 symptoms (body ache, congestion, cough, fever, sinus infection, sore throat), cuts, bruises or may need stitches, earache and infection, injuries (possible broken bones, sprains, dislocations), minor allergic reactions, minor burns, nausea, vomiting or diarrhea without bleeding, abdominal pain or dehydration, minor nosebleeds, pink-eye, red eye, itching, rashes, sexually transmitted diseases with symptoms, sprains and strains, suture/staple removal, urinary, kidney, and bladder infections, and wound check (increased pain, tenderness, redness, swelling or warmth around the cut or injury, fever, or other changes).Services offered include drug screenings, onsite X-rays, physicals, select immunizations, and select laboratory testing.Additional locations listed here Urgent Care locations.

Helps women who are in a life transition prepare for the job market by offering emotional, educational and job search support. Participants receive help with resume preparation, interviewing, information on getting their GED if needed, career search, computer readiness, confidence building and life management. Information about and referrals to appropriate GED, training, college programs and community agencies are also available.

Offers low-cost vaccine clinics. Care may include rabies shots, feline leukemia and feline immunodeficiency virus (FIV), nail trims, and other routine treatments. Microchips are also available.

Provides a 24 hour hotline, support and safe shelter if needed to assist primary and secondary victims of sexual assault. Also offer on site hospital advocates to meet with the survivors to assist in the initial crisis intervention and to answer any questions of the survivor and family members who may be on site. Gives referrals to other agencies within the community. Provides legal advocates to assist in the trial process, if charges are pressed and assist in the filing of the Victims Compensation forms. Also offer community education, training to new sexual assault volunteer advocates and have speakers available.

Mental health services for children and youth, including: individual and family therapy, early childhood assessment and treatment, psychiatric services, case management, residential home for male youth (Paul's House), and comprehensivetreatment to address problematic sexual behavior (Awareness Program). COVID-19 Temporary Message: Signature Health is continuing to offer all services. There are doctors, counselors, case managers, and supervisors on site every day. Hours of operation are 8 am to 5 pm Monday through Friday. Telehealth will be used as much as possible. Individual counseling is provided via telehealth for those who have the capability. Patients who walk in can use a SH tablet to talk with their counselor. If absolutely necessary they will be seen in person. Case managers are available remotely and a Walk-In Case Manager is on site every day. New assessments are being conducted by phone or via telehealth (secure video). Anyone who enters the building will have their temperature taken. If someone brings a patient/client they will be asked to wait outside.

Provides limited emergency financial assistance with housing and utility costs to eligible veterans and their families.

Provide Pack N Play portable cribs and Safe Sleep Survival Kits to families and caregivers that are unable to provide a safe sleep environment due to financial hardship. Survival Kit contains a sheet for the Pack N Play, a sleep sack for the infant, a bedtime storybook and educational materials for the parents. Family must come to Board of Health office to pick up Pack N Play and receive instruction on proper assembly and use before taking Pack N Play home.
