Wic (WOMEN, INFANTS, CHILDREN) | Metrohealth Medical Center

Provides supplemental food card that includes staple foods, fruits and vegetables; nutrition education; breastfeeding education; support and counseling; and referral services to eligible women and children. During the summer also provides a one-time $30 supplemental voucher to a limited number of participants for fruits and vegetables from selected local growers (Farmers' Market Nutrition Program), available while supplies last. Also offers speakers who discuss the program and the value of good nutrition.

Substance Abuse - Residential Treatment | Lake Geauga Recovery Centers

Provides residential substance abuse treatment for people who are in the late stages of addiction. Support and counseling are offered to family members of the individual in treatment.

Moms Quit For Two/Baby And Me Tobacco Free | Friendly Inn Settlement

A Community Health Worker meets 16 times with the client (4 prenatal and 12 postpartum). Each session involves support education and testing to verify continued abstention from tobacco. Program is designed to give women incentives to stop using tobacco during and after pregnancy by providing diapers to those who remain tobacco-free during pregnancy and the first year of their child's life. Provides baby diapers for eligible women. Those who remain tobacco-free will receive 14 $25 vouchers to use for diapers for one year from participation.

Prevention, Retention And Contingency (PRC) - Household Expenses | Tuscarawas County Job And Family Services

Provides assistance with paying for repair or purchase of appliances, personal expenses (clothing, child restraint seats, diapers, toiletries, and so forth), home furnishings (beds, mattress and box springs, kitchen table, chairs, and so forth). Provides assistance with paying for treatment, relocation, and education due to infestation of pest(s) including but not limited to bed bugs, fleas, rodents, roaches. Maximum amount of assistance will vary depending on what is needed.

Bridges To Wellness (BTW) | Access Tuscarawas

Coordinates with other agencies in the community to ensure at-risk-families are receiving as many forms of assistance as possible, such as social, medical and educational services. Community Care Coordinators meet clients in their own homes to assess the family's needs and to establish a care management plan. Targets pregnant women to support throughout their pregnancy to help reduce and eliminate Ohio's Infant Mortality Rate. Offers diapers and wipes to families at all visits by the coordinators.

Neighborhood Navigator | Lorain County Public Health

Provides central intake to women who are pregnant or postpartum to assist in locating resources and coordinate assistance by way of Neighborhood Navigators in the client's preferred setting (home, agency and/or community setting). Performs a detailed screening to identify barriers, needs, and opportunities for women and their families and then a step-by-step approach to action is taken to each item of need. Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid.

Medicaid Applications | Portage County Department Of Job And Family Services

Provides programs administered by the state that provides medical benefits for individuals and families with limited incomes who fit into an eligibility group that is recognized by federal and state law. Programs include the following: - Modified Adjusted Gross Income (MAGI): Medicaid for parents, caretaker relatives, pregnant women, newborns, children, extension adults, transitional Medicaid, and possible dual eligibility with Aged Blind and Disabled Medicaid and Medicare recipients. - Aged Blind and Disabled (ABD): Categorically Needy. Medicaid for 65 and older. Meets Social Security Administration (SSA) definition of blind or disabled. - MPAP: QMB, SLMB, QI-1, QWDI: Medicare Premium Assistance for those in receipt of Medicare. - MBIWD: Medicaid for individuals between ages of 16 and 64 who meet the SSA definition of blind and disabled and are working. Possible premium required. - SRS: Medicaid for those age 21 and over who are receiving SSI or SSDI, meet SRS diagnosis, and are in receipt of another Medicaid category. - Non-Citizen Emergency Medicaid Assistance (NCEMA): Apply at any time. Requires state approval to receive Medicaid eligibility and span. - LTC and Waiver Services: Medicaid for individuals in nursing facilities, assisted living, or home care.
