Also offers speakers who discuss the program and the value of good nutrition.

The HUB is a regional care coordination network focused on improving health outcomes. We partner with primary care, behavioral health, social service providers, and health payers to develop a better coordinated and connected delivery network for low-income residents in our community who are at high risk of poor health outcomes.

Provides central intake to women who are pregnant or postpartum to assist in locating resources and coordinate assistance by way of Neighborhood Navigators in the client's preferred setting (home, agency and/or community setting). Performs a detailed screening to identify barriers, needs, and opportunities for women and their families and then a step-by-step approach to action is taken to each item of need. Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid.

In the program, WIC participants are given $30 in coupons to purchase authorized fresh fruits, vegetables and herbs from authorized farmers at farmers' markets and farm stands.

Provides a residential treatment facility for women suffering from addiction to alcohol or other drugs. Length of stay is determined by medical necessity, with average stay of 4 months. Aftercare referrals provided which may include referral to Intensive Outpatient Program (IOP). Maximum length of in-patient stay is four (4) four months. Two months of aftercare are provided.
