Bridges To Wellness (BTW) | Access Tuscarawas

Coordinates with other agencies in the community to ensure at-risk-families are receiving as many forms of assistance as possible, such as social, medical and educational services. Community Care Coordinators meet clients in their own homes to assess the family's needs and to establish a care management plan. Targets pregnant women to support throughout their pregnancy to help reduce and eliminate Ohio's Infant Mortality Rate. Offers diapers and wipes to families at all visits by the coordinators.

Provides four (4) individualized prenatal quit smoking/vaping sessions to pregnant women. Women are eligible for a $50 voucher for diapers and/or wipes, if they test tobacco-free, during session three and four prenatal and monthly first six (6) months postpartum.

Early Head Start | West Ohio Community Action Partnership

Provides a comprehensive program which, for eligible families, provides case management for pregnant women, infant care, and parenting education. The children are in a group care setting with a low caregiver to child ratio, where they are stimulated by a developmentally appropriate curriculum. They also are directed to needed immunization services and health care. All children are given a developmental screening and referred to supportive services based on the results.

Ohio Equity Institute (OEI) Neighborhood Navigation | Public Health-Dayton & Montgomery County

Our neighborhood navigators help connect pregnant and postpartum women to valuable community resources to support them during pregnancy and up to one year after birth. Our goal is to help support a healthy birth outcome and ensure that infants make it to their first birthday. Our navigators help pregnant people connect to prenatal care and support through home visiting programs, referrals for mental health services, and access to support for smoking cessation and other substance misuse programs. Additionally, our navigators make sure expecting and postpartum mothers have everything they need to welcome their baby home by providing access to baby items, cribs, and other material goods to meet their needs. We also provide referrals to non-baby related resources, as we recognize that the health and well being of the mother can directly impact the health of the child. We can serve any pregnant mother living in Montgomery County, or any mother with an infant under one year of age. —

Provides nutrition education, breastfeeding education and support; supplemental, highly nutritious foods such as cereal, eggs, cheese, milk, whole grain foods, fruits and vegetables, and iron-fortified infant formula which are provided on a WIC Nutrition EBT Card. Also provides referrals to prenatal and pediatric health care and other maternal and child health and human service programs. The WIC Farmers' Market Nutrition Program (FMNP) is a dual purpose program which benefits both WIC participants and farmers. In the program, WIC participants are given $30 in coupons to purchase authorized fresh fruits, vegetables and herbs from authorized farmers at farmers' markets and farm stands.

Provides programs administered by the state that provides medical benefits for individuals and families with limited incomes who fit into an eligibility group that is recognized by federal and state law. Programs include the following: - Modified Adjusted Gross Income (MAGI): Medicaid for parents, caretaker relatives, pregnant women, newborns, children, extension adults, transitional Medicaid, and possible dual eligibility with Aged Blind and Disabled Medicaid and Medicare recipients. - Aged Blind and Disabled (ABD): Categorically Needy. Medicaid for 65 and older. Meets Social Security Administration (SSA) definition of blind or disabled. - MPAP: QMB, SLMB, QI-1, QWDI: Medicare Premium Assistance for those in receipt of Medicare. - MBIWD: Medicaid for individuals between ages of 16 and 64 who meet the SSA definition of blind and disabled and are working. Possible premium required. - SRS: Medicaid for those age 21 and over who are receiving SSI or SSDI, meet SRS diagnosis, and are in receipt of another Medicaid category. - Non-Citizen Emergency Medicaid Assistance (NCEMA): Apply at any time. Requires state approval to receive Medicaid eligibility and span. - LTC and Waiver Services: Medicaid for individuals in nursing facilities, assisted living, or home care.

Pathways Hub | Lorain County Community Action Agency

Offers empowering support to pregnant women throughout their pregnancy and up to their child’s first birthday. The Community Health Worker assists the pregnant woman in navigating the health care system and social services as well as educational support throughout each trimester of pregnancy. Services are individualized and adjusted for each woman's needs. Also provides diapers, wipes, training pull-ups and period products. Sizes vary and may not always be available.
