A Community Health Worker meets 16 times with the client (4 prenatal and 12 postpartum). Each session involves support education and testing to verify continued abstention from tobacco. Program is designed to give women incentives to stop using tobacco during and after pregnancy by providing diapers to those who remain tobacco-free during pregnancy and the first year of their child's life. Provides baby diapers for eligible women. Those who remain tobacco-free will receive 14 $25 vouchers to use for diapers for one year from participation.

Maximum amount of assistance will vary depending on what is needed.

Meets Social Security Administration (SSA) definition of blind or disabled. - MPAP: QMB, SLMB, QI-1, QWDI: Medicare Premium Assistance for those in receipt of Medicare. - MBIWD: Medicaid for individuals between ages of 16 and 64 who meet the SSA definition of blind and disabled and are working.
