Maintains 100 bed facility and provides minimal, intermediate, skilled and subacute nursing care for persons with Alzheimer's and other related dementials. Facility and activities specifically designed for persons with Alzheimer's disease and other related dementias.

Provides public awareness of cancer prevention and early detection. Informs public of the importance of good nutrition, exercise, sun protection, self-examination and clinical screening by distributing educational booklets and brochures, participating in community health fairs and presenting educational workshops.

Includes a three-part series including diabetes education, demonstrations and tasting of healthy foods. Follow up meetings held to discuss progress since the initial class. This is provided by the OSU Extension office.

Provides intensive outpatient chemical dependency treatment in a 3-phase service. In Phase 1, a complete assessment is done, and counseling and group interventions begin. In Phase 2, the focus is on a relapse prevention plan, positive coping skills, responsibility and sober life management. Parenting classes are offered to clients who are required to participate as part of their plan, and are also open to other participants who have children. Phase 3 focuses on self recovery, journaling, and family roles and relationship sessions. 12-step meetings, individual and couples counseling are primary components of the larger program. Program includes education on topics like domestic violence, dealing with trauma, health screenings, referrals to needed basic services (food, housing, etc.) Child care is not available during sessions currently due to COVID 19.

Provides advocacy for health fairs. The American Diabetes Association publishes materials for the general public, as well as, medical professionals. Booths are set-up at public fairs. Books, literature, slides, video, and trained speakers are available. In-Service Training - Offers free training to health professionals, schools, organizations, and corporations. Topics include emergency procedures, diet, nutrition, exercise, complications, and general detection.

Provides monthly telephone contact for people with cancer to listen and talk.

Handmade bags with educational and encouraging items inside given to newly diagnosed women or men who live or work in Champaign County.

Provides breast care services including accurate prevention, early detection, diagnosis, educational programs and support services. Offer Joanie's Promise program which covers the payment of all diagnostic services for women who qualify for financial assistance.

Offers medical, dental and vision services for low-income residents in Columbus. Also provides short-term behavioral healthcare with counseling, group counseling and case management. Recovery supportive services available for those with substance use and addiction issues. Also assists with Medicaid enrollment. Provides HIV/STI testing and other prevention services, including PrEP. Provides HIV care and case management and linkage to care.

Provides various forms of relaxation and stress reduction activities such as yoga, tai chi, reiki and reflexology to help those dealing with cancer cope with the physical and mental stress of cancer. Childcare available with advance notice. Services provided for the individual diagnosed and family/caregiver.

Provides a free phone care consultation program that provides coaching and support for caregivers regularly throughout the dementia journey.

Clients are allowed two cases of Boost nutritional drink per month. A client can also purchase a nutritional drink they may need and submit a reciept for reimbursement of that drink.

Provides health insurance premium payments up to $1,200 for eligible individuals, allowing them to continue existing health insurance policies, thereby receiving adequate prescription drug coverage.

Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Also provides homemaker assistance (meal preparation, laundry and general household management including light housekeeping.) Services also include personal care and homemaking for adults with HIV/AIDS.
