Eligibility
- Serves residents of Medina, Portage, Summit, and Wayne counties.
- Must be living with a chronic medical condition.
- NOTE: Does not require active involvement in medical services.

Completed referral form is preferred.
Hours
- Pathways HUB is available Monday through Friday from 8:00am to 5:00pm.
- Services are community based and location and time of meetings are determined between individual and their CHW.

Application process
- Email or fax completed referral form to Pathways HUB. Referrals accepted from medical professionals, community members, and self-referral. Individuals may also call Pathways HUB for additional assistance.
- Clients can also text "HUB" to 33030 for information, and questions.
Fees
No fee. NOTE: Medicaid will be billed if possible.

Provides adults or children who are living with a chronic medical condition or at risk for a chronic medical condition such as diabetes, high blood pressure, asthma, cancer, and so forth with a Certified Community Health Worker (CHW). CHWs provide care coordination, social services support, and services focused on the social determinants of health in order to improve health outcomes. The CHW will provide assistance with connecting primary and preventative care.
