Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Also provides homemaker assistance (meal preparation, laundry and general household management including light housekeeping.) Services also include personal care and homemaking for adults with HIV/AIDS.

Provides in-home nursing and home health aide services.

Provides a 24/7 crisis hotline for people whose emotional distress may range from difficulty coping with daily pressures to a psychotic episode. Includes immediate assistance for those experiencing suicidal feelings.

Short-term in home respite care is provided as well.

Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Home visits are available for tuberculosis and respiratory patients. For complete homebound patients, home visits are available including, flu and pneumonia vaccines, medication assistance, air conditioner and respiratory equipment (with a physician’s order). Provides an integrated program of home care services designed to address the specific home care needs of those under the age of 60 needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services, and Waiver Benefits. Home Health Services provide nursing, daily living, and skilled therapy. Waiver Benefits provide the same services as Core Services but may also include: home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite care, social work, and care coordination. (Upon turning 60, waiver recipients are automatically transferred onto the Transition II [Carve-out] Waiver, which is only available to these individuals, and cannot be applied for independently).
