Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Provides many skilled nursing services to our clients, including wound care, infusion therapy, supportive services are provided by professional aides and companions who give comfort and support to in-home clients on a day-to-day basis. Our companions will assist with shopping, cooking, reading, errands, light housekeeping and will provide transportation to doctors' visits. Provides an integrated program of home care services designed to address the specific home care needs of those under the age of 60 needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services, and Waiver Benefits. Home Health Services provide nursing, daily living, and skilled therapy. Waiver Benefits provide the same services as Core Services but may also include: home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite care, social work, and care coordination. (Upon turning 60, waiver recipients are automatically transferred onto the Transition II [Carve-out] Waiver, which is only available to these individuals, and cannot be applied for independently).

Homemakers and Home Health Aides are not nurses, but they are supervised by a registered nurse who will help assess the Veteran's daily living needs.
