A team of health care professionals, including nurses, nurse aides, social workers and dietitians, as well as physical, occupational and speech therapists, will work with the personal physician to help restore, promote, or maintain a person's health. Such assistance helps prevent the need for a trip to the emergency room or admission to the hospital.
