Provides short or long term non-medical assistance with daily living to seniors, new or expectant mothers, individuals recovering from surgery, prone to falling, living with Alzheimer's or dementia, or having limited mobility. Live in care 24 / 7 is available.

Provides non-medical in-home support for older adults and individuals who need assistance with daily activities. Services may include healthcare duties, light housekeeping, respite care, companion care. Participates in the Ohio Medicaid Waiver Program, PASSPORT, Senior Levy services, and Veterans programs.

Home Health Aides to stay with person needing minimal personal assistance but some help with bathing, meal preparation or light housekeeping, errands, 24 hour minimum but no minimum on days used. Can request service on occasional or regular basis. We will fit the hours and program to the client's needs. Supervised by RN at no cost with RN on call 24 hours a day. Also can make visits every two weeks or more often if necessary.

Provides a case manager helps ensure an individuals needs are met with home and community based services. These services are delivered through various case management programs and through a network of providers and may include but not be limited to help with personal care, housekeeping, preparing or receiving home delivered meals, transportation, adult day care and home medical equipment.

Provides home nursing and health services (including physical, occupational, and speech therapy), and post-surgical care. Also provides personal care assitance, housekeeping, transportation, errand running, laundry help, and companionship.

Helps disabled older adults remain safe and independent at home. Prevents unnecessary nursing home placements. Consumer-Directed option gives consumers the option to serve as employer by hiring eligible individuals of their choice to provide their personal care services.

Program provides Nursing, Aide care, Physical Therapy, Occupational Therapy and Speech Therapy. Home Health provides both rehab and long term care in the patient's home.

Provide a higher level of personal care, shaving, bathing, skin care, catheter care, colostomy care, turning and positioning, assistance with ambulation, transfers to wheelchair, etc. Nurse aides can do vital signs and remind patient to take medication as set up by RN. Available minimum of 2 hours up to 24 hours a day by 8 hour shifts. Supervised every 2 weeks by RN. RN on call 24 hours a day. Plan of treatment submitted to physician. Also offer companion and homemaker service.

Provides skilled home nursing, physical/occupational/speech therapies, home health aides and personal care. Also provides psychiatric nursing in the home. Also provides hospice care.

Provides home health care which includes medication management and reminders, personal care and bathing assistance, meal planning and preparation, grocery shopping, errands and light housekeeping and laundry.

Provides skilled nursing and home health care including professional nursing assessment, medical services such as blood pressure checks, medication monitoring, blood sugar checks, wound care, catheter care, and tube feeding, behavioral health care (limited to dementia and depression), home health aide services, homemaker and companion services, temporary illness support programs, and rehabilitation services including physical, occupational, and speech therapy. Short-term in home respite care is provided as well.

Provides skilled nursing services

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides homemaking and personal care services to medically challenged adults, helping them maintain independent living in their own homes. A homemaker can assist with light cleaning, laundry, preparing a light meal, and general errands. Home Health Aids can assist with bathing, observing and monitoring health status, mouth and skincare, and respite care.

The Ohio Home Care Waiver Program (OHC) meets the needs of people who prefer to get long-term care services and supports in their home or community, rather than in an institutional setting. CareSource has contracted with the Ohio Department of Medicaid in the role of a Case Management Agency. CareSource in partnership with local Area Agency on Aging 3 will deliver case management services for the OHC program to help people meeting Medicaid financial criteria, who are under 60 years of age and assessed with an intermediate or skilled level of care, along with other program requirements.

Provides a range of home health care services including but not limited to skilled nursing services for diabetic and cardiac care, as well as assistance for older adults and those with a chronic illness or disability. Offers personal care and homemaker assistance, including bathing and grooming, light housekeeping, laundry, assistance with daily living tasks, companionship, meal planning and preparation, errand running, and transportation/escort to places such as medical appointments, shopping, and family functions.
