Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Home visits are available for tuberculosis and respiratory patients. For complete homebound patients, home visits are available including, flu and pneumonia vaccines, medication assistance, air conditioner and respiratory equipment (with a physician’s order). Provides in-home services for adults 60 and older as an alternative to nursing home care. A social worker or nurse will visit the client to assess needs and create an individualized plan. Services may include adult day services, home delivered meals, personal care, medical equipment, minor home repairs/modifications, and homemaker services such as house keeping.

The concept of CHSP is to provide clustered services to residents in the housing complex so that they can have a more individualized care plan. For example, if a person needs assistance with getting up in the morning, or a medication reminder, they can receive that service, and then be visited again later in the day for additional services if needed. The services are provided by a contract agency. The cluster model of services provides the residents the benefit of consistent staff, and a care plan that is tailored to meet their individual needs.

Also can make visits every two weeks or more often if necessary.
