Provides short or long term non-medical assistance with daily living to seniors, new or expectant mothers, individuals recovering from surgery, prone to falling, living with Alzheimer's or dementia, or having limited mobility. Live in care 24 / 7 is available.

Provides non-medical in-home support for older adults and individuals who need assistance with daily activities. Services may include healthcare duties, light housekeeping, respite care, companion care. Participates in the Ohio Medicaid Waiver Program, PASSPORT, Senior Levy services, and Veterans programs.

Home Health Aides to stay with person needing minimal personal assistance but some help with bathing, meal preparation or light housekeeping, errands, 24 hour minimum but no minimum on days used. Can request service on occasional or regular basis. We will fit the hours and program to the client's needs. Supervised by RN at no cost with RN on call 24 hours a day. Also can make visits every two weeks or more often if necessary.

Provides a case manager helps ensure an individuals needs are met with home and community based services. These services are delivered through various case management programs and through a network of providers and may include but not be limited to help with personal care, housekeeping, preparing or receiving home delivered meals, transportation, adult day care and home medical equipment.

Provides help to older adults who want to remain independent and living in the community. Enrollees receive personalized services that are coordinated by a care manager. The care manager can assist with things like disease management, health insurance benefits, service coordination and transitional care. Services may include adult day care, chore, homemaker, home delivered meals, personal care, safety monitoring and minor home repair.

Provides home nursing and health services (including physical, occupational, and speech therapy), and post-surgical care. Also provides personal care assitance, housekeeping, transportation, errand running, laundry help, and companionship.

Helps disabled older adults remain safe and independent at home. Prevents unnecessary nursing home placements. Consumer-Directed option gives consumers the option to serve as employer by hiring eligible individuals of their choice to provide their personal care services.

Assists persons with disabilities to move from a long term care facility to a community setting.

Offers at home care by licensed nurse practitioners and professionals. Services include physical, occupational and speech therapy, medicine administration and social work services to help client find more permanent assistance.

Provides skilled nursing and therapies in the home.

Provides an integrated program of home care services designed to address the specific home care needs of those under the age of 60 needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services and Waiver Benefits. Home Health Services provide nursing, daily living, and skilled therapy. Waiver Benefits provide the same services but may also include: home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite care, social work, and care coordination. (Upon turning 60, waiver recipients are automatically transferred onto the Transition II [Carve-out] Waiver, which is only available to these individuals, and cannot be applied for independently)

Provides home care, palliative care, and hospice services. Also provides grief support, including groups, counseling, camps, and events.

Program provides Nursing, Aide care, Physical Therapy, Occupational Therapy and Speech Therapy. Home Health provides both rehab and long term care in the patient's home.

Provide a higher level of personal care, shaving, bathing, skin care, catheter care, colostomy care, turning and positioning, assistance with ambulation, transfers to wheelchair, etc. Nurse aides can do vital signs and remind patient to take medication as set up by RN. Available minimum of 2 hours up to 24 hours a day by 8 hour shifts. Supervised every 2 weeks by RN. RN on call 24 hours a day. Plan of treatment submitted to physician. Also offer companion and homemaker service.

Provides care to veterans through services including: Home-based Mental Health Care; Home-based Primary Care and dietary services.

Provides a 24/7 crisis hotline for people whose emotional distress may range from difficulty coping with daily pressures to a psychotic episode. Includes immediate assistance for those experiencing suicidal feelings.

Provides services to older adults in their homes as an alternative to nursing home care. PASSPORT services may include personal care/respite, homemaker, adult day care, home delivered meals, chore service, minor home modifications, adaptive devices, home medical equipment, pest control services, transportation, social work, counseling, emergency medical response systems, independent living assistance, and community transition services.
