Connects individuals and families to support services and area agencies to assist with overcoming health and/or social needs barriers they are experiencing.
Provides transportation services to seniors needing to go to medical appointments, adult day care, restaurants, grocery stores, human service appointments, banks, and other essential activities. Preference is given to those who need rides medical appointments. There may be limitations for those who need a ride for general errands (such as grocery shopping, etc).

Seeks to safeguard individuals through effective mediation assuring access to care, maintenance of employment and preservation of their financial stability relative to their diagnosis of life threatening or debilitating diseases.
Provides social work services for those utilizing Veterans Affairs medical facilities. Social Workers are available to assist in a variety of settings to include the primary care clinic, during hospitalization, at Emergency Department visits, in mental health and rehabilitation units, and upon enrollment in specialty medical programs. Will assist the veteran, their family members, and Caregivers in resolving housing, finance, mood, relationship, and physical limitation issues in individual, group, or family treatment, whichever suits your needs. Social workers often provide long-term case management services to veterans who are at high risk of being admitted to a hospital, those who have very complex medical problems, and those who need additional help and support.

Provides temporary living residents for families of patients admitted to OSU Medical Center. Offers eight fully-furnished apartments. Families are responsible for their own food, paper products, cleaning and laundry supplies.

The HUB is a free service where you are partnered with one community health worker who will walk alongside you to help connect you to the local resources you need to become stronger and healthier. The HUB is a regional care coordination network focused on improving health outcomes. We partner with primary care, behavioral health, social service providers, and health payers to develop a better coordinated and connected delivery network for low-income residents in our community who are at high risk of poor health outcomes.

Offers a dental discount program for Franklin County residents. With the Dental Discount Program, a county resident schedules an appointment with a participating dentist or specialist and presents the membership card to receive a discount at the time of service. Members can receive 5-50% on dental procedures including routine oral exams, cleanings, dentures and crowns. There are no forms to complete after the initial enrollment and no limit on the number of times a member can use the card to receive discounted dental services during the year. Everyone in the household is eligible to receive a discount. The Dental Discount Program can work in conjunction with health savings accounts, flexible spending accounts and health reimbursement arrangements.
