The Social Services Department provides services to patients of Joint Township District Memorial Hospital including discharge planning, referral and linkage to community resources, counseling regarding health related life style changes, and assessment of ongoing psychosocial needs.

The LTC Ombudsman advocates for the rights of people living in nursing homes, assisted living, adult care homes or using long-term services and supports in the community in Butler, Clermont, Clinton, Hamilton and Warren counties. Home Options also offers ombudsman objective information to help with choosing a care provider in Southwest Ohio.

Hispanic Health program designed to help the Hispanic community have access to medical care, social services and health education. Provide blood sugar, cholesterol and blood pressure screenings. Health Liaison serves as the link between the Hispanic community and health services. Assistance given to clients in Spanish with appointments and interpretations. The Promotora (community health liaison) teaches basic health education topics in Spanish.

Acts as a mediator, investigating and attempting to resolve any concerns regarding MetroHealth, including the main site, satellite locations, nursing facilities, and those representing MetroHealth in the community. All problems will be thoroughly investigated in cooperation with the department involved, and the results will be relayed to, and discussed with, the complaining party.

Provides medical air transportation for cancer patients to access care facilities.

Provides temporary housing for patient (active duty, veteran, or family member), families, or care giver accompanying patient being treated at Wright Patterson AFB and Dayton VA Medical Centers.

Provides information and referral, counseling, crisis intervention, and discharge planning to patients of the hospital.

Offers a membership program through several community clinics that gives eligible clients of those agencies discounted access to specialists at participating local hospitals. Client will still receive all primary medical services through their clinic. Discounts only apply to necessary specialist visits as referred by the client's clinic-based general practitioner. If already a client of one of the clinics, can enroll in the plan through that clinic at any time. All new clients will be automatically eligible as part of the clinic enrollment process.

Provides answers to patient questions about their present and future accounts, billing, or insurance. Also screens for any possible outside solution including, Marketplace insurance or Medicaid, before offering payment assistance or payment plans.

Community Health Centers of Greater Dayton provides access to high quality, affordable, primary healthcare in the region with general health education, physician referrals, and medical social work.

Connects individuals and families to support services and area agencies to assist with overcoming health and/or social needs barriers they are experiencing.
