Fees

No fee for CHAP membership; fixed fee; however,sliding scale may be available for specialist medical services. Individual providers set discounts/rates independently of CHAP.

Service area

Cuyahoga County, Ohio
Offers a membership program through several community clinics that gives eligible clients of those agencies discounted access to specialists at participating local hospitals. Client will still receive all primary medical services through their clinic. Discounts only apply to necessary specialist visits as referred by the client's clinic-based general practitioner. If already a client of one of the clinics, can enroll in the plan through that clinic at any time. All new clients will be automatically eligible as part of the clinic enrollment process.
