Provides residential treatment for veterans with chronic medical and/or psychiatric diagnosis. Length of stay is negotiable. The treatment plan is tailored to help individuals function as independently as possible.
Offers a 208-bed domiciliary for veterans who require medical care who do not need but are capable of living independently. Residents have their own room, with cable access, and a shared bath. Features two reading lounges, a computer lounge, recreational facilities, clubs and regular and special activities.

Provides a State of Ohio establishment comprised of two facilities - a home located in Sandusky, Ohio (approximately 60 miles west of Cleveland) and a home located in Georgetown, Ohio (approximately 45 miles east of Cincinnati). The Georgetown Home is a licensed nursing home providing two levels of nursing care – standard care and special care (Alzheimers/dementia). The Sandusky Home, in addition to offering standard care and special care in its licensed nursing home, also offers Veterans Hall (DOM), a domiciliary for those who are able to function in an independent living situation, and DOM+ for those requiring very limited assistance (supervised care) but who do not require the level of care provided to nursing home residents.

Provides a 30-bed, drug-free, alcohol-free, and smoke-free transitional housing facility with support services for homeless male Veterans where each Veteran has their own bedroom and share common areas. Provides laundry facilities, onsite therapeutic groups through the expressive arts as well as individualized comprehensive case management. Offers two hot, home cooked meals and a continental breakfast each day. Provides limited transportation.
Provides care for ambulatory adults. Services for non-ambulatory persons with independent transfer skills may be considered. Individual bedrooms do not include a private bathroom so residents must be able to get themselves down the hall to use the shared bathroom (dormitory-style). Capacity is 63 residents. All meals served in central dining room. Approved Veteran's Group Home. Nurses and the Resident Care Attendants are on duty around the clock. Residents must have the ability to adapt to a daily routine with minimal direction. Director of Nursing performs pre-admission screenings to determine if this facility can adequately meet the medical, social, and emotional needs of applicants. Not able to accept Medicaid or Medicare for payment of room, board, and care.
