Provides case management services to young adults. Clients will be paired with a trained case manager who will help develop a personalized case plan to foster independence. Participants will learn skills such as financial planning, job readiness, study skills, time management, goal-setting, problem-solving and critical thinking. Ten transitional housing beds are reserved for Geauga County residents only (clients may access case management services even if they are not using the housing portion of this program).

Assist homeless advocate organizations with program, resources, funding development, training and technical assistance. COHHIO operates some programming for families and youth. COHHIO also has a team that provides leadership around the Homeless Management Information System and the Balance of State (80 rural counties) Continuum of Care.

Operates as a collaborative of agencies that work with unsheltered people. Takes reports of where homeless people are and then goes out to that location to help them find needed services such as shelter and treatment. Accepts reports such as: group of homeless people have been hanging out on the street; homeless person wandering in the neighborhood; homeless person sleeping behind house. This is not an emergency service. Does not handle reports of violent or criminal activity.

Produces a comprehensive list of the telephone numbers, and in many cases addresses, of shelters, meal sites, public health facilities, legal services, and other service providers in Cuyahoga County which assist the homeless in emergency situations. List is updated annually.

Provides strategic planning, coordination, and development of resources serving homeless persons in Cleveland and Cuyahoga County. Can provide information to the public on aspects of the continuum of care for homeless persons. Is available to present talks to groups about the issues of homelessness. Does NOT provide direct services to the homeless.

Provides a resource where eligible individuals can request non-perishable food, water, hygiene products, and other items such as: first aid kits, and clothing (including some winter items: gloves, hats, etc.) to homeless youth. Volunteer counselors work with youth to help solve life problems.

Provides a Community Resource Center that offers information about wrap-around support services, coordinated transportation services, on-site community resources, peer support and a rapid re-housing program.

Provides planning and coordination of homeless services in Geauga County.

Provides housing navigation and supportive services for youth and young adults who are experiencing homelessness. Participants in the program will be assigned to a Navigator that will assist them in finding stable housing as well as aiding, supporting, and linking them to other community resources based upon their individual needs. Navigators can meet with youth and young adults at locations convenient to them on either the East or West side of Cleveland.

Provides speakers, films, and slide presentations to the general community on topics regarding the homeless population.

Has 3 vans that go out into the community and provide homelessness outreach, including hot meals and clothing.

Works to protect the rights of the homeless: monitors to assure that municipalities do not unfairly target the homeless or broadly enforce laws which would be harmful to the homeless; assists homeless persons who may have been the victims of hate crimes; coordinates information exchange regarding the rights of the homeless. The agency has experts on homeless voting available to assist with registration, get out the vote or in-person identification issues. Holds forums at shelters and drop-in centers to uncover problems, and responds with advocacy, letter-writing, and media attention to such problems.

Works to coordinate planning to address the problem of homelessness, and studies proposed solutions, including recommendations of outreach workers and the homeless themselves. Available to make presentations to community groups regarding homelessness, often with a homeless or formerly homeless person participating in the presentation. Hosts regular meetings with homeless people to inform advocacy position. Publishes regular updates on public policy issues.

Collaborates to bring together diverse organizations to work as a system, rather than as a fragmented set of resources; Employs an outcome-based funding model that measures performance, monitors programs’ success, and assures the system’s effectiveness as a whole; Combines innovative solutions and best practices with time-tested strategies to implement programs that quickly and stably house people in crisis.

Publishes a periodic newspaper containing articles, poems, and editorials regarding issues of homelessness. The newspaper helps to educate the public about homelessness, and also provides job opportunities for people to return to the workforce by buying the papers for a low price and reselling them to the public. The vendors receive training, including being informed of city ordinances regarding selling, are in some cases assigned shifts at major distribution points, and have the opportunity to publish experiences of being without housing.
