Social Workers are available to assist in a variety of settings to include the primary care clinic, during hospitalization, at Emergency Department visits, in mental health and rehabilitation units, and upon enrollment in specialty medical programs. Will assist the veteran, their family members, and Caregivers in resolving housing, finance, mood, relationship, and physical limitation issues in individual, group, or family treatment, whichever suits your needs. Social workers often provide long-term case management services to veterans who are at high risk of being admitted to a hospital, those who have very complex medical problems, and those who need additional help and support.

Veterans And Families First Program | Volunteers Of America - Ohio And Indiana

Provides supportive services to low-income veterans and their families living in or transitioning to permanent housing. Offers outreach, case management, health care referrals, legal assistance referrals and help with fee payments, daily living costs and service referrals, housing counseling services, personal financial planning services, transportation expense assistance and assistance in obtaining VA and other benefits. Additionally, the program may also provide time-limited payments to third parties (ex. landlords, utility companies, moving companies and licensed child care providers) if these payments help veteran families stay in or acquire permanent housing on a sustainable basis.

Provides Patient Advocates to assist clients of the Veterans Affairs (VA) health system that need help getting care or getting their problems resolved. Clients should first bring suggestions or concerns regarding their to their treatment team (treatment team includes your doctor, nurse, social worker, dietitian, pharmacist, chaplain, therapist and other professionals associated with the client's medical care). If the client does not feel their concerns are not being addressed by their treatment team, they should contact a VA Patient Advocate.

Home Repairs | Lake-Geauga Habitat For Humanity

Assists low to moderate income homeowners with paying for repairs and rehabilitation on their homes. Depending on eligibility and the availability of funding, agency may be able to offer this program at no cost to the homeowner (usually if they are age 65 or older or a veteran). In some instances, a zero-interest loan with an affordable repayment schedule may be offered as well. Examples of the types of work that the program can help with are: painting, siding, roofing, doors, steps, wheelchair ramps, sub floors, carpeting, etc. Does not help with foundation. Most work is provided by volunteers, however some work may be provided by contractors. This is not an emergency service; the process can take 12-24 months (however, smaller jobs may be completed sooner).

Soldiers Relief | Montgomery County Veteran'S Services

Provides emergency financial assistance to Montgomery County resident veterans who qualify and establish a bona fide need for the following assistance: utilities (electric, gas and water) food (foodbank) mortgage or rent payment and burial benefits. Veterans may be able to receive up to $1,000 to go towards their insurance deductible for the tornado assistance.

Contacts deployed military personnel on behalf of families experiencing an event which requires an urgent communication such as a death, illness, or birth of a child. Assists with emergency leave verification and basic information and referral. Provides access to emergency funds available through the various military aid societies (Army Emergency Relief, Navy Marine Corps Relief Society, Air Force Aid Society, and the Coast Guard Mutual Assistance). Assists with acquiring funds which can be used for a variety of purposes such as burial of a loved one, emergency travel, and emergency food and shelter. NOTE: Assistance may come in the form of a loan or grant.

Horses For Heroes Program | Breaking Free Therapeutic Riding Center

Offers a program to Veterans who are interested in working with or around horses or in a farm and barn atmosphere. The Horses for Heroes program will allow Veterans to identify their own goals for the program. To work individually or in a group setting with their horse partner and an instructor. The first day at the barn is spent becoming acquainted with the horses. One can progress from there to learning to groom and saddle a horse to riding and going on trail rides. We have two paths to choose from: non-mounted equine care, handling, and understanding or mounted recreational riding and lessons with an instructor. Whether you want to work with horses or just help out around the farm, Breaking Free Horses for Heroes has a place for you. There is no need for previous experience with equine; our staff will work with you to learn as much or as little as you want about equine. Once finished with the program Veterans are encouraged to return and help future Veteran's coming into the program.

Provides free emergency suicide crisis care for veterans at any VA or non-VA healthcare facility including inpatient or crisis residential care for up to 30 days and outpatient care for up to 90 days. Veterans do not need to be enrolled in the VA system to use this benefit. Expenses will be free, paid for, or reimbursed.
