Transports veterans for free to Washington, D.C. to visit memorials dedicated to honor their service.

Provides transportation to the VA Pittsburgh Healthcare System Hospitals at University Drive and the H.J. Heinz Campus in Aspinwall. Transports to the Louis Stokes VA Hospital System at Cleveland (Wade Park) along with its Community Based Outpatient Clinics (CBOC) in Akron, Canton and New Philadelphia. NOTE: VA Medical Centers in Cleveland and Pittsburgh.

Offers general mental health counseling and for specific issues: sexual trauma counseling related to military service (military sexual assault), post traumatic stress disorder due to combat service, veteran reintegration counseling, couseling for victims of spousal/partner abuse, and bereavement couseling. Counseling may be offered in individual and group settings. Does not deal with legal issues.

Offers nursing home / assistive living care for veterans who require intermediate care as well as special care for veterans with Alzheimer's disease and other forms of dementia. Also provides hospice care for terminally ill veterans.

Offers counseling services to veterans who have served in a combat zone or experienced sexual trauma. Services include individual, group and family or marital counseling. Offers sexual trauma, drug and alcohol and bereavement counseling. Offers assistance with benefits and employment guidance.

Provides social work services for those utilizing Veterans Affairs medical facilities. Social Workers are available to assist in a variety of settings to include the primary care clinic, during hospitalization, at Emergency Department visits, in mental health and rehabilitation units, and upon enrollment in specialty medical programs. Will assist the veteran, their family members, and Caregivers in resolving housing, finance, mood, relationship, and physical limitation issues in individual, group, or family treatment, whichever suits your needs. Social workers often provide long-term case management services to veterans who are at high risk of being admitted to a hospital, those who have very complex medical problems, and those who need additional help and support.

Provides supportive services to low-income veterans and their families living in or transitioning to permanent housing. Offers outreach, case management, health care referrals, legal assistance referrals and help with fee payments, daily living costs and service referrals, housing counseling services, personal financial planning services, transportation expense assistance and assistance in obtaining VA and other benefits. Additionally, the program may also provide time-limited payments to third parties (ex. landlords, utility companies, moving companies and licensed child care providers) if these payments help veteran families stay in or acquire permanent housing on a sustainable basis.

Provides gravesite tending services for deceased Veterans including replacing broken headstones and gravesite maintenance such as cutting grass, trimming overgrowth, and removing litter.

Provides nutritional assessment and dietary recommendations for optimal health of the patient.

Provides professional behavioral health therapy to children, adults and families through horses, counseling and nature.

Loans crutches, walkers and sometimes wheelchairs to those who need them. Availability of items will depend on what the agency has on hand.

Provides Patient Advocates to assist clients of the Veterans Affairs (VA) health system that need help getting care or getting their problems resolved. Clients should first bring suggestions or concerns regarding their to their treatment team (treatment team includes your doctor, nurse, social worker, dietitian, pharmacist, chaplain, therapist and other professionals associated with the client's medical care). If the client does not feel their concerns are not being addressed by their treatment team, they should contact a VA Patient Advocate.

Assists low to moderate income homeowners with paying for repairs and rehabilitation on their homes. Depending on eligibility and the availability of funding, agency may be able to offer this program at no cost to the homeowner (usually if they are age 65 or older or a veteran). In some instances, a zero-interest loan with an affordable repayment schedule may be offered as well. Examples of the types of work that the program can help with are: painting, siding, roofing, doors, steps, wheelchair ramps, sub floors, carpeting, etc. Does not help with foundation. Most work is provided by volunteers, however some work may be provided by contractors. This is not an emergency service; the process can take 12-24 months (however, smaller jobs may be completed sooner).

Provides emergency financial assistance to Montgomery County resident veterans who qualify and establish a bona fide need for the following assistance: utilities (electric, gas and water) food (foodbank) mortgage or rent payment and burial benefits. Veterans may be able to receive up to $1,000 to go towards their insurance deductible for the tornado assistance.

Provides a program to help recently-discharged veterans make a successful transition from military life to civilian life. The program is peer-guided in connecting transitioning service members to community-specific resources at their desired location. While being involved in this program veterans are able to request monetary assistance with housing, utilities, food, and clothing.

Offers an identification card to Veterans that serves as an alternative to the Armed Forces discharge record known as a DD Form 214 or DD214. The Veteran Identification Card serves as a convenient way to provide proof of military service and may be honored by Ohio merchants who give Veterans a discount. Businesses that honor the identification card for Veterans do so voluntarily and choose the savings they offer.

Assists veterans in filing claims, applying for benefits, and receiving emegency assistance as necessary.

Contacts deployed military personnel on behalf of families experiencing an event which requires an urgent communication such as a death, illness, or birth of a child. Assists with emergency leave verification and basic information and referral. Provides access to emergency funds available through the various military aid societies (Army Emergency Relief, Navy Marine Corps Relief Society, Air Force Aid Society, and the Coast Guard Mutual Assistance). Assists with acquiring funds which can be used for a variety of purposes such as burial of a loved one, emergency travel, and emergency food and shelter. NOTE: Assistance may come in the form of a loan or grant.

Provides an online tool designed to help veterans discover and understand their aptitudes, and abilities; provides certification opportunities for several job categories; and connects job seekers to local in-demand jobs. There are a limited number of slots available for this program.

Provides transportation to and from Veterans Affairs (VA) medical facilities for Veterans (Cleveland VA Medical Center, Parma VA Outpatient Clinic, Akron VA Outpatient Clinic, Canton VA Outpatient Clinic) and Compensation and Pension (C&P) Exams fo Veterans unable to arrange transport on their own.

Provides primary care, general physical examinations, dermatology, and podiatry. Some sites offer geriatric assessments. Geriatric Evaluation is a health care visit with a team of experts that results in a treatment plan aimed at helping Veterans maintain good health, improve quality of life, reduce the need for hospital stays, and reduce the need for long term care services.

Offers a program to Veterans who are interested in working with or around horses or in a farm and barn atmosphere. The Horses for Heroes program will allow Veterans to identify their own goals for the program. To work individually or in a group setting with their horse partner and an instructor. The first day at the barn is spent becoming acquainted with the horses. One can progress from there to learning to groom and saddle a horse to riding and going on trail rides. We have two paths to choose from: non-mounted equine care, handling, and understanding or mounted recreational riding and lessons with an instructor. Whether you want to work with horses or just help out around the farm, Breaking Free Horses for Heroes has a place for you. There is no need for previous experience with equine; our staff will work with you to learn as much or as little as you want about equine. Once finished with the program Veterans are encouraged to return and help future Veteran's coming into the program.

Provides a two-building, maximum security county jail for the Cuyahoga County Court of Common Pleas and some suburban courts. Also provides a veterans jail housing unit which includes benefits such as: weekly specialized and targeted programming, camaraderie of other veterans, job development planning, services to avoid homelessness, access to mental health services (including substance abuse and dual diagnosis) and veteran mentors.

Provides very limited transportation expense assistance for Veterans for auto repairs and payments on a case-by-case basis.

Provides free emergency suicide crisis care for veterans at any VA or non-VA healthcare facility including inpatient or crisis residential care for up to 30 days and outpatient care for up to 90 days. Veterans do not need to be enrolled in the VA system to use this benefit. Expenses will be free, paid for, or reimbursed.
