Assists with connection to family supports such as respite, camps, social activities, coaching, parent education, advocacy and assistance for paying the cost of these services. Transition planning provided between home to residential placement and back to the home and community. This includes determining the goals of placement, reviewing progress made and supports needed to bring the youth home and assure a stable environment. Structure includes a Service Coordination Steering Committee that reviews cases, approves use of funding, and monitors outcomes.

Provides leadership to assure an effective system of collaborative, coordinated and efficient community services which assist each family and child to meet their individual needs and responsibilities. Administers the following: * Kinship Care Closet * Service Coordination * Care Management

Crisis intervention services and emergency shelter for families with dependent children who have been referred by the homeless system coordinated point of access. Those entering receive basic needs, access to school and child care, case management and referrals to end their housing crisis within an average of 42 days.

Investigation of alleged child abuse, neglect or exploitation within families or child care facilities, schools, group homes, etc. Alternative intake can also do assessments for cases of domestic violence if children are in the home and affected. Note a case must be opened in order to receive additional services.

Works with families, youth, and partner agencies providing service coordination for multi-system youth. Provides information on targeted programs and services.
