Assists children with special health care needs. Coordinates services, links with quality care and financial assistance for medical care and provides identification and referral of children with potentially handicapping conditions. Offers service coordination, advocacy, and public health nurse home visits to assist families in obtaining and coordinating services.

Conduct facility inspections and issue related licenses in following areas: food services and food establishments, rental property, weed and high grass control, tattoo establishments, mobile home parks, solid waste facilities, infectious waste facilities, schools, and nuisance complaints.

Provides daycare to children ages 4 weeks to 17 years old who have disabilities in Montgomery County and other surrounding counties.

Offers after school activities for youth ages 6 to 17 who have developmental disabilities. Which include arts and crafts, outside play, reading, movies and homework assistance. During the Dayton Public Schools calendar school year schedule.

Offers summer camping activities to children with disabilities ages 6 to 17 for 10 weeks of the summer. In addition, they offer a "Super Senior" Summer Camp for those 18 to 20 with disabilities who are stll in High School.

Provides assistance to children with special health care needs and their families through three different programs with varying eligibility. *Diagnostic Program: services for up to six months, from CMH-approved providers, to rule out or diagnose a special health care need or establish a plan of treatment. Services may include: Tests and x-rays Therapy evaluations Public health nursing services Visits to CMH-approved doctors Up to five days in the hospital *Treatment Program: services for one year, from CMH-approved providers, for treatment of an eligible condition. May include: laboratory tests, x-rays, physician visits, prescription medications, medical equipment and supplies, surgeries, hospitalizations, public health nursing services and therapies as well as service coordination. All services must be related to the child's eligible condition. *Service Coordination Program: helps families locate and coordinate services for their child.

The Employment Services program offers a broad range of assessment, training and job placement services and provide on-on-one support to reach personal employment goals. Work Assessments offer opportunities for "job tryouts" with community-based employers to determine areas of interest and existing skill sets. Job Development focuses on the outcome of community job placement. Individuals may receive assistance with resume development, interview proficiency, job search techniques and employer advocacy. Job Coaching assists individuals to develop the skills to be successful in community employment by offering supplemental training and oversight to solidify job performance. Transition Services assist students, ages 14-22, with preparation for employment via career exploration and job readiness training. Job club, Summer Youth and Project SEARCH programs were designed specifically to achieve the employment goals of students transitioning into the workforce. We also provide Job Readiness and Career Exploration classes for Jr. High and High School students from throughout our service area.

Services include: - Family Navigator Program provedses screening, information, referral options, follow up services to families with special needs children. - Pathways to a Healthy Pregnancy offers coordination services for Medicaid eligible pregnant women; services for teen health issues, including pregnancy.
