Provides assistance to locate, activate and coordinate a variety of in-home services needed by frail elderly persons age 60 and older who are at risk of entering a nursing home. This program provides short-term acute care services to assist with Personal care, homemaking and meals following Hospitalization or while Waiver services are being set up.
