Community Care Coordination | Ohio District 5 Area Agency On Aging

Provides assistance to locate, activate and coordinate a variety of in-home services needed by frail elderly persons age 60 and older who are at risk of entering a nursing home. This program provides short-term acute care services to assist with Personal care, homemaking and meals following Hospitalization or while Waiver services are being set up.

Passport Home Based Waiver Program | Ohio District 5 Area Agency On Aging

Provides comprehensive in-home, health-related services to 60+ individuals who meet an Intermediate Level of care. Provides Medicaid and state funds to assist and enable an individual with long-term care needs to remain in their own home. Services paid for by PASSPORT and provided by agencies that PASSPORT contracts with. Some of the services available under this program are: personal care, homemaker, adult day care, transportation, medical supplies, home-delivered meals, and nutrition counseling.
