Includes benefits such as Medicaid, Medicare Part D, SSI, SSD, SNAP (food stamps), HEAP (LIHEAP), Golden Buckeye Card, Exemption Prescription Assistance programs, housing and others. Will assist clients with filling out and submitting applications and forms and provides follow-up with client. Visits seniors and adults with disabilities in the home, where they provide client with combined utility assistance application, assist client, to fill it out, mail it, and follow up as needed regarding LIHEAP. Makes presentations to senior groups regarding benefit programs and provides assistance completing applications.

Provide environmental prevention strategies to the community of Mahoning County to reduce youth substance use, and adult misuse over time through a community coalition of key stakeholders. Sponsor prescriber training to address opioid misuse, disseminates literature via media, neighborhood canvassing to reduce youth use, establishes drug free zones throughout the community, facilitates Safe Home campaigns, sponsors and creates youth ambassadors who are champions in efforts to reduce peer substance use, monitors retail outlets for underage sales.

Provides information and outreach to the public about oral health. Oversees various programs that promote dental care and good oral health.

The QRT team will connect with individuals who have experienced an overdose, illicit drug event, or are struggling with addiction, and their family to provide information, assist with treatment options, and referral. Education on the use of Narcan, a prescribed opiate overdose reversal medication, will be given and a free Narcan kit will be provided.

Provides full-time outreach to homeless individuals who may be in need of services. The Outreach Team visits shelters, camps, drop-in sites, and other places where homeless individuals frequent, providing basic medical care and support. Individuals with more complicated issues may be directed to a clinic for follow-up or more intense services.

Provides information and referral of social services and benefits information to older adults. Will provide assessments in the home for residents when needed. Offers counseling to help seniors cope with life-style changes, personal, marital, and family problems. Assist with forms completion and maintains a registry of chore service providers

Provides prevention education on domestic violence to the community. Offers consultations and education to community groups, schools, and churches; trainings for medical and mental health professionals; and Domestic Violence in the Workplace programs for central Ohio employers.

Annual event raises awareness of heart disease as the number one killer of women. Offers networking, health fair and luncheon. Event teaches women about living a longer, healthier life and also raises funds for research and education. This initiative serves as a catalyst for change to improve the lives of women globally.

Provides speakers on cancer prevention and early detection to organizations. Builds relationships with community-based organizations and health care providers, schools, parks and communities to plan, organize and develop effective cancer prevention and early detection programs and activities. It is recommended Cervical Cancer testing (screening) should begin at age 25; Breast Screenings/Mammogram should start every year for Women between age 40-44; and regular mammograms should happen annually for Women between age 45-54. Women 55 and older can switch to a mammogram every other year.

Provide temporary shelter for single women and women with children who are homeless. Length of stay is 30 days with goal of moving to stable housing. Assistance with finding permanent housing and other resources.

Referrals can be from a variety of sources such as family, friends, ER, law enforcement or the user themselves.

Protects community health by providing anonymous and confidential HIV testing (OraSure/Oraquick) and counseling, and encouraging behavioral changes that reduce the risk of AIDS, other sexually transmitted diseases and tuberculosis. Provides an 18 foot van designed to raise awareness of the Healthy Start Program, the program's expanded income eligibility standards, and to identify and enroll eligible children for no-cost medical care. In order to draw people to the vehicle at the event site, and to entertain children while their parents are learning about the program, Healthy Start giveaways are offered to amuse the children. These include literature, coloring books and crayons, magnets, grab bags, lanyards, sports bottles. Also offers cards with Healthy Start emergency numbers.
