Provides social services to families and individuals experiencing barriers with accessing public benefits and community resources. Services include (but are not limited to) public benefit application assistance, resource education, referrals to access community resources, and replacement of vital documents.

Coordinates a collaboration of Cleveland Hearing and Speech Center and Help Me Grow, which works to ensure that families of deaf and hard of hearing children receive the specialized support they need. Provides each family with the ongoing support of a certified outreach specialist who provides complete, impartial information on the condition as well as on the many options for communication and therapy, as well as information on the Deaf Community. Provides links to speech and language therapy, school programs, and community resources.

Provides outreach to people experiencing mood disorders due to issues related to aging, psychological problems, and other issues affected by the aging process. Services include geriatric assessment, case management, counseling (group and individual), medication monitoring, in-home safety assessments and other services determined by case managers. Services to women include attention to psychiatric issues related to physical stages in their lives.

Offers cultural competency training and community awareness campaigns. Support for LGBTQ survivors of hate and bias violence, hate crimes, intimate partner/domestic violence, sexual violence, and stalking.

Provides outreach and assessment services for severely mentally disabled persons who are currently homeless. Offers outreach to the streets, shelters and other places the homeless gather; helps clients stabilize and make the transition into community support programs. May take client to shelter, detox, or other community resource.

Operate an alcohol and drug prevention program. Licensed by the State of Ohio. Implements evidence based prevention programs using best practices curriculum at area public and private schools, re-entry services for youth at the Mahoning County Juvenile Justice Center, and targets the community at large using environmental prevention strategies. Aim is to reduce substance misuse across all age groups, and reduce recidivism in youth under the jurisdiction of Mahoning County Juvenile Court.

Provides outreach through information packets and presentations to inform community members about services and local resources.

Provides full-time outreach to homeless individuals who may be in need of services. The Outreach Team visits shelters, camps, drop-in sites, and other places where homeless individuals frequent, providing basic medical care and support. Individuals with more complicated issues may be directed to a clinic for follow-up or more intense services.

Provides free legal assistance to victims of crime, including direct representation in the criminal justice system to uphold crime victims' rights. Does not perform civil work (i.e. divorce or child custody matters). Also provides informational and technical assistance to attorneys, victim service providers and legislators on crime victims' rights laws.

Identifies and attempts to introduce individuals who are homeless and experiencing mental illness or substance use disorders into the social services/ behavioral healthcare service delivery system. Services include case management and referrals to other supportive services. The Homeless Outreach Center offers showers, laundry facilities, and a telephone for homeless individuals to use.

Outreach services provide older adults with the assistance needed to access the human services system and provide linkage between client and needed services. This service includes management of a home delivered meal program, telassurance, public benefits assistance, housing placement assistance, friendly visiting, and assessments

The QRT team will connect with individuals who have experienced an overdose, illicit drug event, or are struggling with addiction, and their family to provide information, assist with treatment options, and referral. Education on the use of Narcan, a prescribed opiate overdose reversal medication, will be given and a free Narcan kit will be provided.

Annual event raises awareness of heart disease as the number one killer of women. Offers networking, health fair and luncheon. Event teaches women about living a longer, healthier life and also raises funds for research and education. This initiative serves as a catalyst for change to improve the lives of women globally.
