Provides information and referral about needed social services and will provide outreach services in the home for seniors and those who are shut-ins. Offers assistance with completing various forms including help with simple wills, living wills, Power of Attorney for Health Care, etc.

Kids Healthmobile | Cuyahoga County Job And Family Services

Provides an 18 foot van designed to raise awareness of the Healthy Start Program, the program's expanded income eligibility standards, and to identify and enroll eligible children for no-cost medical care. In order to draw people to the vehicle at the event site, and to entertain children while their parents are learning about the program, Healthy Start giveaways are offered to amuse the children. These include literature, coloring books and crayons, magnets, grab bags, lanyards, sports bottles. Also offers cards with Healthy Start emergency numbers. Serves as a resource for people who are in need of services, but who do not know how to access them.

Ryan White Title I Program | Cuyahoga County District Board Of Health

Administers federal funding that provides medical, dental, and social support services to uninsured and underinsured persons living with HIV/AIDS. Facilitates the federally mandated Cuyahoga Regional HIV Services Planning Council, which determines community HIV health care needs, service priorities, and service funding allocations. Produces a Community Resource Guide listing medical, dental, and social service resources available to those living with HIV/AIDS in Ashtabula, Cuyahoga, Geauga, Lake, Lorain, and Medina Counties.

Coalition For Health Promotion | Youngstown Urban Minority Alcoholism And Drug Abuse

Provide environmental prevention strategies to the community of Mahoning County to reduce youth substance use, and adult misuse over time through a community coalition of key stakeholders. Sponsor prescriber training to address opioid misuse, disseminates literature via media, neighborhood canvassing to reduce youth use, establishes drug free zones throughout the community, facilitates Safe Home campaigns, sponsors and creates youth ambassadors who are champions in efforts to reduce peer substance use, monitors retail outlets for underage sales.
