The Special Need Alert Form is for a friend or family member that suffers from Alzheimer's, Dementia, Autism, Mental Health or Special Needs. If there is an emergency at the residence, first responders will be notified that an individual resides there who has filled out the form and the information will be relayed to first responders prior to their arrival.
