Plans for and implements health care events which provide free health care, dental care, vision care and educational assistance to uninsured and underinsured Ohioans. Works with a network of local agencies, free clinics, hospitals, private companies, healthcare professionals and programs in Ohio to provide care.

Plans, develops, and evaluates mental health and alcohol and drug services in Montgomery County. .

Service Coordination can work with families to help identify goals and work with local agencies to find appropriate services for families. WRAP gets together all service providers to work on goals the family creates. Ages prenatal through age 21 and live in Hancock County. Client should be involved in more than one system (i.e. JFS, Court, Wic, School, Etc.)

The DAAP coalition works together to ensure the community-wide prevention of alcohol abuse, the use of illegal drugs, and the misuse of prescription/over the counter medicines. DAAP's mission is to bring drug and alcohol awareness to the Morrow County community, through educational programs, speakers, and organized events. Current projects include: Hidden in Plain Sight: Hidden in Plain Sight brings awareness to parents/guardians in an interactive exhibit of a teenager's bedroom, that helps identify signs of risky behaviors in teens. Our goals for the presentation are to inform parents/guardians and other adult participants on how to have a conversation, not a confrontation, with teens regarding substance abuse, signs and symptoms of substance abuse, and local resources for assistance.Medication Disposal Bags: FREE medication disposal bags with the two locations to drop off unused/expired medications in Morrow County.Medication Lock Boxes: FREE medication lock boxes for Morrow County residents. The goal is educate in safe medication storage and disposal to lower the misuse/abuse/accidental overdose of medications, and to lower the improper disposal of medications. Medication Disposal Pouches: FREE Dispose of any unused/expired medications in the pouch provided by adding water and sealing it. The bags break down the medication and are safe for the environment. The pouch is biodegradable.Parents Who Host, Lose the Most: brings awareness to parents/guardians about the health ans safety risks regarding underage drinking through a campaign around youth events such as dances, graduation, and other school events.

Develops financial resources for human services through an annual campaign and other programs such as planned giving, Combined Federal campaign, and year-round communication about United Way services and community needs. Accepts donations of cash and payroll deduction, monies, and may accept donations of stocks and other selected items.

Members are health professionals, parents, educators, elected officials, merchants, business members, police, administrators, and students who are concerned about alcohol, tobacco, and drug use among school-age children in Trumbull County. ASAP Opiate Task Force meets monthly. Focus are youth education, identifying neighborhoods experiencing high rates of drug related crimes and overdoses, providing community education, and providing education to medical professionals.

Facilitates the implementation of criminal and juvenile justice system improvements through coordination of funding initiatives. Advises elected officials and agencies on the needs, priorities, and allocation of resources in the criminal and juvenile justice systems.

Delaware County Family and Children First Council strives to enhance the well-being of our children and families by building community capacity, coordinating systems and services, and engaging families.

CUCF collaborates with multiple agencies that have volunteer opportunities and also contracts with community members to deliver wraparound services and parent mentoring. Call coordinator if transportation barriers exist. Monthly meeting on the odd months 3rd Thu 2:30pm-4pm at 940 London Ave.

Provides unmatched services, resources, support and counsel to help individuals achieve their charitable giving goals. Assists other non-profits by funding initiatives and offering expertise to help them operate more efficiently. Helps build a better Dayton by identifying important issues and bringing people and organizations who can solve them.

Provides information and resources to those dealing with food insecurity in the Fairfield County community. Coordinates, funds, and provides education to low-income families and individuals in our community about their ability to eat healthy on the limited funds they receive through the Supplemental Nutrition Assistance Program (SNAP) and provides education and training opportunities to everyone in the community in regards to hunger and hunger related health issues.

Provides prevention education, develops agency protocols to assists human trafficking victims, and provides grant funding opportunities for community service providers.

A family that is receiving services from more than one public agency or a family with multiple needs may benefit from an individual family service coordination plan. Such a plan identifies and organizes services for a family and its children. The plan names the individuals responsible for parts of the plan. Services included in the plan may be provided by public or private agencies or by informal supports such as neighbors, other family members, churches, clubs and community organizations. Families play an active role in writing the plan and share responsibility for carrying out the plan. The plan may be referred to as a Wraparound Plan.

Provides land use, zoning, development, and other community, regional, environmental and solid waste planning services. Focuses on the areas of economic and social conditions, land use, transportation systems, utility and community systems, public and private facilities and services, environmental control, natural resource development and conservation, long range programming, and financing of capital improvement projects.

Functions as a network of social service providers and funders that plan and coordinate services for fathers. Promotes the importance of fathers through radio and television along with distributing promotional materials and sponsoring special events. Offers pamphlets and flyers about the Fatherhood Initiative that agencies can distribute to clients.

Family and Children First is a partnership of government agencies, community organizations and parents committed to improving the well-being of children and families through strategic coordination of resources. This partnership is designed to draw people out of their day-to-day systems and align resources and activities around a shared vision that Ohio's families and children will thrive and succeed.

Administers federal funding that provides medical, dental, and social support services to uninsured and underinsured persons living with HIV/AIDS. Facilitates the federally mandated Cuyahoga Regional HIV Services Planning Council, which determines community HIV health care needs, service priorities, and service funding allocations. Produces a Community Resource Guide listing medical, dental, and social service resources available to those living with HIV/AIDS in Ashtabula, Cuyahoga, Geauga, Lake, Lorain, and Medina Counties.

Assesses community needs and problems, monitors service delivery systems, and fosters development of new approaches to respond to community needs. Also sponsors an annual county-wide fundraising campaign.

Contracts with the community network and community support services to provide mental health and substance abuse services to Greene County residents.

Planning and monitoring of county behavioral health services. Administer public funds for prevention and treatment services for mental health and alcohol/drug recovery. Promote healthy behavior, support rehabilitation, and advocate for recovery from mental illness and alcohol/drug addictions.

Provides a systematic improvement process with respect to human services planning and development.

The Clark County Family and Children First Council is assisted by the Clark County Child and Family Collaborative in the design, coordination and implementation of a system of care for children and youth and their families.

Coordinating services; Case/Care Management for Children's Issues,Case/Care Management Referrals, Children's Rights Groups, Issue Advocacy for Families and Children, Planning/Coordinating and Advisory Groups for Children's Issues, Planning/Coordinating and Advisory Groups for Youth Issues through legislation advocating for children with multiple needs and their families. Children and Family First Council offers a variety of different programs for families including the Help Me Grow Program which serves families with children pre-natal through age 3 with home visits and assistance with developmental delays.
