A family that is receiving services from more than one public agency or a family with multiple needs may benefit from an individual family service coordination plan. Such a plan identifies and organizes services for a family and its children. The plan names the individuals responsible for parts of the plan. Services included in the plan may be provided by public or private agencies or by informal supports such as neighbors, other family members, churches, clubs and community organizations. Families play an active role in writing the plan and share responsibility for carrying out the plan. The plan may be referred to as a Wraparound Plan.
Assess the needs and resources for community child care programs. Consults with community leaders, organizations and businesses on child care issues. Maintains up-to-date information on child care needs and resources. Actively promotes legislation and initiatives to address key child care and early education issues.

Seeks to improve the functioning of children and to preserve the integrity of families in the community through a collaborative system for multi-need, multi-system families. Can involve all major public and private agencies as needed. Focuses on issues such as mental health and substance abuse, abuse and neglect, juvenile justice, developmental disabilities, youth employment, health and wellness, and school readiness.
Plans and coordinates community services for citizens with developmental disabilities with the goal of eliminating segregated placement of individuals and returning those in institutions to community life. Monitors community programs and facilities to ensure access and quality of services.

Coalition led by the Trumbull County Mental Health and Recovery Board. Members are health professionals, parents, educators, elected officials, merchants, business members, police, administrators, and students who are concerned about alcohol, tobacco, and drug use among school-age children in Trumbull County. ASAP Opiate Task Force meets monthly. Focus are youth education, identifying neighborhoods experiencing high rates of drug related crimes and overdoses, providing community education, and providing education to medical professionals.
