Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid.

Uses a voucher system to provide the items to clients. Offered on an emergency basis, as approved by staff, to others.

Diapers

Offers a free, voluntary support program for expectant and new mothers. Participants receive 1:1 support from a Support Specialist through in-home or community-based visits. Support specialists provide connections to community resources, pre- and post-partum education, and offer guidance related to early parenting.

Coordinates with other agencies in the community to ensure at-risk-families are receiving as many forms of assistance as possible, such as social, medical and educational services. Community Care Coordinators meet clients in their own homes to assess the family's needs and to establish a care management plan. Targets pregnant women to support throughout their pregnancy to help reduce and eliminate Ohio's Infant Mortality Rate. Offers diapers and wipes to families at all visits by the coordinators.

Offers free pregnancy testing and ultrasounds. Earn While You Learn class "Understanding Pregnancy." Free new baby merchandise, for example onesies, bottles, sleepers, handmade blankets, diapers & wipes. Complete all 5 sessions receive a car seat or stroller.

Surrounds pregnant and new mothers with support, education and resources in a weekly group program to ensure a healthy pregnancy and a healthy baby. Services include prenatal and child birth education, medical referrals, talking with doctors and nurses, parenting resources, food assistance, baby items, as well as a healthy meal and the opportunity to socialize with other pregnant moms. Childcare and $5 Kroger gift card at each meeting. Transportation provided if needed.
