Provides non-medical in-home care. Services include companionship, meal preparation, light housekeeping, errand and transportation services, daily telephone reassurance, laundry, and clothing shopping, emergency monitoring services, dressing and grooming assistance.

Provides short or long term non-medical assistance with daily living to seniors, new or expectant mothers, individuals recovering from surgery, prone to falling, living with Alzheimer's or dementia, or having limited mobility. Live in care 24 / 7 is available.

Provides participants with a structured day of activities in a safe environment; breakfast, lunch and snacks are provided; for persons with Alzheimer's, a related disorder, or frail elderly. Transportation available.

Provides care and services to patients with life-limiting illnesses and their families. Care is provided in a variety of settings, such as, private homes, extended care and assisted living facilities, hospitals, and two Hospice House locations.The focus is on pain-symptom control, preparation/support before and after death, and bereavement support for families.

Provides comprehensive home health and personal care services including: skilled nursing services, infusion therapy, pediatric services, women's health services, medical social services, private duty services, home care aides, homemakers/companions, nutritional counseling, oncology services and wound care.

Provides homemaker/personal care (HPC) services for adults with disabilities to maintain independence. Assistance is provided with household chores and personal care such as getting dressed and keeping clean; making food; cleaning and doing laundry; getting out into the community and going to medical appointments.

Provides short-term temporary relief for caregivers and their families. Offers in-home care with trained staff experienced in special needs of developmental disabled individuals ages 6 months through elderly.

PASSPORT provides help at home for seniors as an alternative to nursing home care. Senior services available include homemaker, transportation, personal care services, home-delivered meals, emergency response systems, adult day services, and case management. PASSPORT, providing help at home for Ohio seniors, is also referred to as Ohio's home and community based Medicaid-waiver program for older adults.

This program addresses two specific issues within the senior population in Franklin County: 1) Loneliness and isolation and 2) underuse of resources active seniors have to give. The program engages two different populations of seniors: home-bound seniors and active, low-income seniors. Senior Companions, all of whom qualify as low-income, receive stipends and travel reimbursement to visit home-bound seniors for 15+ hours a week. They provide needed socialization and non-medical assistance so clients can remain independent in their homes. The program helps both groups stay connected and retain their independence.

Provides homemaking and personal care services to medically challenged adults, helping them maintain independent living in their own homes. A homemaker can assist with light cleaning, laundry, preparing a light meal, and general errands. Home Health Aids can assist with bathing, observing and monitoring health status, mouth and skincare, and respite care.

Provides assistance in the home with basic personal care and home management tasks including housekeeping, grooming, meal preparation, and laundry. Serves those who are 18 years of age and older and have a disability.

Offers assistance on a day-to-day basis allowing client to maintain independence. Services include personal care assistance, transportation,and Alzheimer's caregiver respite.

Provides personal care attendants as well as errand-running assistance and light housekeeping services for homebound seniors.

Provides skilled nursing care that includes personal care and home health aides but only if skilled nursing is main service needed. Specialized services for cardiac, diabetic, pulmonary, and wound care are also available.

Provides personal care services that combine a variety of health care and custodial care services in the home; some services include bathing, dressing, meal preparation, laundry, homemaker duties, errands, all aspects of personal care, and companion and support services; also may include nursing services, such as medication set-up, medication reminders and medical assessments.

Provides food for those who need it. Food boxes include fresh produce, hygiene supplies and $5.00 voucher towards purchases at Springfield Goodwill Stores.
