Provides short or long term non-medical assistance with daily living to seniors, new or expectant mothers, individuals recovering from surgery, prone to falling, living with Alzheimer's or dementia, or having limited mobility. Live in care 24 / 7 is available.

This program addresses two specific issues within the senior population in Franklin County: 1) Loneliness and isolation and 2) underuse of resources active seniors have to give. The program engages two different populations of seniors: home-bound seniors and active, low-income seniors. Senior Companions, all of whom qualify as low-income, receive stipends and travel reimbursement to visit home-bound seniors for 15+ hours a week. They provide needed socialization and non-medical assistance so clients can remain independent in their homes. The program helps both groups stay connected and retain their independence.

Provides palliative pain and symptom management, care, and emotional support for persons with a life-limiting illness (life-threatening or terminally ill) and their families. Provides supportive services such as bereavement counseling, social work, art and music therapy, and spiritual care. Respite care is provided for client's families.

Provides in-home assistance with personal hygiene, housekeeping tasks, food preparation, transportation, and other personal care needs for older adults.

Provides homemaking and personal care services to medically challenged adults, helping them maintain independent living in their own homes. A homemaker can assist with light cleaning, laundry, preparing a light meal, and general errands. Home Health Aids can assist with bathing, observing and monitoring health status, mouth and skincare, and respite care.

Offers home health aide and home nursing services, including RN/LPN, case management, medication and disease process management, patient/caregiver education, IV therapy, wound management, ostomy care, home health aide services, physical therapy and speech therapy. Also offers personal care and light housekeeping.

Provides a range of home health care services including but not limited to skilled nursing services for diabetic and cardiac care, as well as assistance for older adults and those with a chronic illness or disability. Offers personal care and homemaker assistance, including bathing and grooming, light housekeeping, laundry, assistance with daily living tasks, companionship, meal planning and preparation, errand running, and transportation/escort to places such as medical appointments, shopping, and family functions. Also provides additional support, such as rehabilitative therapies and social work services.

Services may include homemaker assistance, personal care, light housekeeping, respite care, companion care, senior care, and transportation. Participates in the Ohio Medicaid Waiver Program, PASSPORT, Senior Levy services, and Veterans programs.

Self-directed home care support program. Eligible veterans are approved for annual budget that can be used to hire/purchase services to help avoid nursing home placement. Types of services provided include: * Personal care tasks * Meals or other home delivery services * Respite care * Care coordination * Goods and services to improve home safety * Case management provided by the Area Agency on Aging * Provided through collaboration with the Louise Stokes Cleveland VA Medical Center.

Provides assistance with personal care and housekeeping tasks. Services may include bathing, showering, bed baths, light housecleaning, laundry, blood pressure checks, food preparation and cooking.

Provides homemaker/personal care (HPC) services for adults with disabilities to maintain independence. Assistance is provided with household chores and personal care such as getting dressed and keeping clean; making food; cleaning and doing laundry; getting out into the community and going to medical appointments.

Provides personal care services that combine a variety of health care and custodial care services in the home; some services include bathing, dressing, meal preparation, laundry, homemaker duties, errands, all aspects of personal care, and companion and support services; also may include nursing services, such as medication set-up, medication reminders and medical assessments.

Provides the following services to anyone in need, regardless of his or her ability to pay: * Medication for pain and symptom management, * Professional nursing visits, * Assistance and instruction to family caregivers, * Personal care and homemaker assistance, * Medical and social services, * Spiritual care from the hospice chaplain, * Dietary guidance from a registered dietitian, * Physical, occupational, speech and music therapies, * Trained volunteers, * Bereavement support, * Short term inpatient care, * On-call professional staff, and a supplemental paid caregiver fund.

Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Offers showers, Clothing and laundry by appointment. All supplies are provided i.e soap, shampoo, towels, laundry soap, by the Dream Center. A hot meal is provided while waiting on appointment.
