All county boards of developmental disabilities in Ohio provide local Medicaid authority for the administration of developmental disability waivers. The developmental disability waivers are a Medicaid service that help people served by county boards of developmental disabilities to live in their own homes, shared homes and family homes and received services within the community. Federal funds pay approximately 60% of the cost of waivers, with the remaining 40% of funding being paid by the county board of developmental disabilities through local tax levies or allocated state funds. A waiting list assessment is completed to determine the need for such services. There are three types of Medicaid waivers administered by the Allen County Board of Developmental Disabilities: Level One Waiver, Self-Empowered Life Funding (SELF) Waiver, and Individual Options Waiver. Each Medicaid DD waiver is unique in regard to funding and program requirements. Service and Support Associates (SSAs) are the primary point of contact for each person enrolled on a Medicaid waiver.

Provides short or long term non-medical assistance with daily living to seniors, new or expectant mothers, individuals recovering from surgery, prone to falling, living with Alzheimer's or dementia, or having limited mobility. Live in care 24 / 7 is available.

Provides participants with a structured day of activities in a safe environment; breakfast, lunch and snacks are provided; for persons with Alzheimer's, a related disorder, or frail elderly. Transportation available.

Offers health and safety-related independent living improvements such as handrail and grab bar installation, ramp repair, and other services and health and safety improvements including window air conditioner installation, unclogging drains and more.

Provides onsite septic system inspections and consultations, issues sewage permits, and investigates sewage complaints

Program provides services to assist with the transiting out of nursing facilities.

The supported living program offers individualized instruction and assistance to individuals with developmental disabilities to teach the specific daily living skills needed by the individual to live in a safe, healthy environment. CDS is an Individual Options Waiver and Level 1 Waiver provider through the county board of developmental disabilities system. Transportation available to those in program. Self-pay is also accepted.

Provides seniors with information and assistance in filling out forms which may be difficult to understand and complete correctly. Also assists Medicare recipients to understand and register for Part D (prescription plans).

Neighborlink Miami Valley volunteers provide leaf raking, lawn care, (including mowing, hedging and weeding) and general yard cleanup services (including trash and other debris removal) for people who need or want assistance.Visit I Need Help for more information and to sign up for assistance.

Provides shared living and other supported living services to individuals with developmental disabilities. Services range from providing shared living and social recreational activities to teaching the individual to maintain a home independently.

Offers an ex-offender employment program matching potential clients with low-cost, supervised services from individuals striving to successfully re-enter society after being incarcerated. Low-cost services include lawn care, leaf removal, and snow removal.

Assists with completing and submitting forms for needed social services, such as government benefit programs.

Emergency or accessibility home repairs. Focus on structural and accessibility repairs such as, roofs, gutters, windows, HVAC, electrical, handicap accessibility ramps and bathroom installations. Currently have a wait list, must call for eligibility and to be placed on the wait list.

Provides snow removal for senior homeowners in Brooklyn.

Providing outreach and advocacy services to older adults in the oxford area.Including Medicare/Prescription Drug Plans, OPERS Connector assistance, other benefits services; Adult Day Program Centers, Arts and Crafts Instruction, Audiological Evaluations,Benefits Screening, Bereavement Support Groups, Blood Pressure Screening, Caregiver/Care Receiver Support Groups, Community Meals, Congregate Meals/Nutrition Sites, Dance Instruction, Disease/Disability Information, Driving Evaluation, Exercise Classes/Groups, Field Trips/Excursions, Food Pantries, Food Safety Education, General Recreational Activities/Sports, Home Delivered Meals, Homemaker Assistance, Housing Search Assistance, Medicaid Applications, Outreach Programs for Older Adults, Quilting Clubs, Recreational Activities/Sports, Senior Centers, Senior Olympics,Speakers/Speakers Bureaus, Specialized Information and Referral to older adults in the oxford area.

Offers homemaker service to clients ages 60 and over. Homemakers come to the home and perform light housekeeping, laundry, grocery shopping, necessary errands, etc.

Provide Mahoning County residents that qualify with a stove, refrigerator, washer, dryer, bedding, and/or a crib. Services beginning April 1, 2022, through December 31, 2022.

Provides in-home assistance with personal hygiene, housekeeping tasks, food preparation, transportation, and other personal care needs for older adults.

Offers a variety of in-home care services for eligible older adults, including personal care, transportation, care management, meals, etc.

Provides financial assistance for minor home repair projects to low-income seniors and individuals who are permanently physically disabled. Eligible work may include, but not be limited to: electrical (replace fuse or light bulb, repair/replace a switch, repair/replace frayed cords/plugs, etc.); plumbing (repair/replace toilets and sinks, leaking faucets, drain and supply lines, vent pipes and sump pumps, etc.); masonry (repair sidewalks, porch floor and steps; apply waterproofing compound to basement walls; minor tuck-pointing, etc.); carpentry/maintenance (repair doors, jambs, frames, hinges, window frames, kitchen cabinets, decking, floors, porches, holes in walls; install glass; install smoke detectors; install deadbolt locks; etc.); and heating and air conditioning (repair/replace furnace and air conditioner).

Offers home repairs and rehabilitations to income-eligible Lorain County homeowners. Repairs may include roofs, water heaters, furnaces, A/C, electric panel, electrical, ADA modifications, plumbing, and more. The Home Repair Program provides no-cost upgrades and fixes for qualified homeowners. This grant-funded program requires no repayment by the homeowner. The Home Rehabilitation Program offers more extensive repairs and upgrades for homes. Assistance will be in the form of a declining-forgivable loan. There is no monthly payment, and no interest will be charged. No credit check is needed. 100% of the amount borrowed will be forgiven over (5) years provided owner continues to own and occupy the home. NOTE: Funding varies from year to year and might not always be available.
