Provides information and assistance in completing forms to apply for various benefits and entitlements they may be eligible for. Also provides basic information about income taxes, as well as State, Federal, and local Income Tax forms. Provides blood pressure screening and one on one evaluations from the agency staff, assessing needs around food, personal care, and other urgent matters.

Provides connection to a service that will mow yards and pick up leaves seasonally for eligible residents.Service includes edging driveways and walkways, collecting and disposing of grass clippings and general trimming around trees and shrubbery.

Provides various water repairs. For the main water line, will repair or make service line replacements. In the home, will repair or replace water tanks in addition to repairing plumbing fixtures such as leaking faucets, broken toilets (installation of water saver toilet in the event of non-repairable toilet). Also, repair/correction of frozen water lines and k-line replacements. When available, may install water conservation kit to help reduce water usage.

Provides: home health care (home health aides and home nurses) by medical staff; homemaker services; and durable medical equipment sales and rentals.

Provides grants to assist homeowners with home repairs. The following repairs may be eligible for the program: --Building envelope structural repairs, including the roof, floors, ceiling, walls and foundation; --Roofing repair and/or replacement; --Gutter repair and /or replacement as well as other moisture mitigation work; --Heating system repair and/or replacement; --Plumbing repair and/or replacement; --Septic system repair and/or replacement; --Electrical upgrades, repair or replacement; --Interior/exterior repairs and alterations to allow handicapped mobility and accessibility, such as kitchen and bathroom modifications, ramps, handrails, and grab bars. Repairs are provided by contractors hired by COAD.

We help first time mothers have healthy pregnancies, improve child health & development, and become more economically self-sufficient.

Provides support to those navigating the refugee/immigration process such as helping clients with filling out and submitting documents related to their refugee status and/or immigration case (such as applying for green cards, various immigration papers, and/or travel documents). Gives advice on test preparation, what to expect during the process, and what questions to expect during interviews with government officials. Provides basic information on how to prepare for obtaining employment (filling out applications, what to wear, how to handle job interviews). Provides translation assiatnce (English to Somali). Does not give legal advice (but will refer the client to the appropriate agencies if they need it).

Provides house painting; constructs exterior ramps to the home. Provides tree trimming and maintenance of large trees that may present a hazard on the homeowner's property.

HWAP is a residential energy efficiency program that reduces energy use by reducing air infiltration and heat loss in homes. CenterPoint Energy Home Weatherization Program - includes upgrading insulation or if needed installing a new natural gas furnace.

Provides one time emergency heating assistance to income-eligible households that are disconnected, threatened with disconnection, need to re-connect service that has been shut off, need to transfer or establish new service, or have less than a 25% supply of bulk fuel. Maximum amount of assistance is $175 for regulated utilities, $450 for wood and coal, $750 for unregulated utilities, $900 for bulk fuel or up to $1500 for furnace repair. Customers can also receive combined benefits for any of the above. The utility company will delay the disconnection until five business days after the confirmed appointment.

Provides in-home assistance with personal hygiene, housekeeping tasks, food preparation, transportation, and other personal care needs for older adults.

Provide highly individualized, coordinated system of services and supports for people with disabilities who do not require 24-hour care to facilitate their ability to live in their own homes or apartments. Services may include homemaker or personal care attendant.

Provides assistance with minor home repairs, snow removal, lawn mowing, leaf raking, grocery shopping, picking up medicine, running errands, etc.

Outreach program providing counseling/case management and essential services enabling eligible individuals to live independently in the community. Services may include care coordination, counseling/case management, assistance with filing reparations and/or pension claims; financial assistance due to eligible individuals; and personal care/light housekeeping. Volunteers are specially trained to provide social companionship to survivors. Also provides an opportunity to gather and socialize, to celebrate Jewish holidays, and to attend arts and cultural programming on a regular basis.

Offers snowplowing for older adults age 65 and over.

Provides homemaker assistance.

Matches volunteers with seniors who require assistance with errands, chores and minor home repair. Will also installs smoke detectors as needed.

Provides maintenance of streets, street signs, and traffic signals. Offers weed abatement and clean up of ditches in front of residents' yards. Provides snow removal for seniors and disabled residents.

Provides information and referral about needed social services and will provide outreach services in the home for seniors and those who are shut-ins. Offers assistance with completing various forms including help with simple wills, living wills, Power of Attorney for Health Care, etc.

Assistance with home energy conservation. Will conduct energy inspection of heating appliances and insulation. Provides energy conservation information. Will weatherize home and provide for replacement of furnaces as funding is available. Will not replace roofs.
