Case Management provides individualized interventions that include skill development, advocacy, linkage to medical services, housing, benefits, social activities and other community resources.
Provides people with Alzheimer's an identity bracelet, wallet ID cards and clothing labels, and enrolls them in a national database of law enforcement agencies nationwide. When an individual wanders away, family members or caregivers can call a toll-free number to alert the computer network.
Provides assistance in meeting the costs of medicine and emergency needs when no other means are available. There is a limit of $400/calendar year ($200/semi yearly) for assistance with medication grants. There is a one-time/year amount of up to $100 for emergency grants. Emergency needs may include, but are not limited to, utilities and car repair. Offers transportation assistance in the form of mileage reimbursement, bus passes, and cab vouchers (on an emergency basis) for dialysis and transplant patients. Makes available personalized medical alert tags. No assistance is offered for rent, mortgage, or lease payments. Also offers financial assistance to those with kidney disease who are pursuing higher education.

Provides medical identification bracelets and necklaces and a 24-hour emergency response service. Some of the services offered include: an identification emblem (necklace or bracelet) engraved with their medical condition that is recognized by emergency and medical professionals; a 24-hour phone number; a wallet card (additional fee) and seat belt ID holders (additional fee).

Health programs include: health education and screening (from a medical team); hearing aid clinic, emergency alert. Support groups include Alzheimer’s Caregiver, Multiple Sclerosis, grandparents, Healing Hearts (Grief support lead by Grief Recovery Specialist.
