Basic healthcare information and a listing of current medications is kept in a slim pouch on the refrigerator door. A sticker is placed on the entry door to the house to alert personnel in the event of an emergency.

Provides for the safety of elderly individuals by giving them a red magnetic pouch to keep on their refrigerator containing vital information needed which would alert paramedics, ambulance attendants and other emergency personnel.

The form provides emergency crews with the individual's personal information, along with their special needs information including known triggers and behaviors, communication abilities and calming methods

Offers "file of life" kits for Upper Arlington residents, in which medical information is stored in a magnetic pouch on the outside of a resident's refrigerator. This information is accessed by Emergency Medical Services in case of emergency.
