We provide support services for eligible individuals age 3 through adult with eligibility determinations, areas of transition, coordination of benefits, resource links, access to funding, community opportunities, provider search, education, advocacy and employment.

Provides behavioral and mental health services including: assessment services, individual and group mental health case management services, individual and group behavioral health counseling and therapy services, pharmacological management, targeted case management, crisis intervention and psychological testing.

Community Treatment Teams and designated case manager services attend to the health and social needs of persons with serious and persistent mental illness and their families. Intensive case management approach for persons ages 18 and older.

Offers medical, dental and vision services for low-income residents in Columbus. Also provides short-term behavioral healthcare with counseling, group counseling and case management. Recovery supportive services available for those with substance use and addiction issues. Also assists with Medicaid enrollment. Provides HIV/STI testing and other prevention services, including PrEP. Provides HIV care and case management and linkage to care.

Provides psychiatric assessment and treatment, nursing services and support services. Teams include a clinical leader, nurse, psychiatrist and community support services staff to interact with clients. Also offers assistance with entitlements, housing, recreational activities, job search and daily living skills.

The Passport Program provides in-home alternatives to nursing home care for low-income seniors. The Programs are administered through local area agencies on aging.

A home care program that assists low income persons (age 60 and older) who are eligible for nursing home placement to remain in home in their community. Arrangements are made for services such as home health aide, home delivered meals, adult daycare, homemaking, and participant directed services etc. Call (800) 521-6419 to schedule an appointment for assessment.

Treats those experiencing mental health and/or substance use issues via our experienced team of licensed providers. We do not turn anyone away seeking help, regardless of your ability to pay.

Senior services include help with citizenship or naturalization, support for medical and legal appointments, social outings with peers, and more.

Provides services to promote family stability; family service plan development, case management and direct services to families

Provides mental health and case management services to families with children who are having difficulty in school or at home, have poor coping skills and have mental, emotional and behavioral disturbances. Offers education and consultation through the linkage of community support services.

Provides information and referral service, placement and coordination, crisis intervention, advocacy, major unusual incident reporting and investigating, and service monitoring for agency clients. In addition, assists families with special expenses that may be involved with having a developmentally disabled family member at home.

Provides behavior and mental health services to youth up to age 21. Services include assessment, case management, individual and group counseling, pharmacaological management, crisis intervention and psychological testing.

Provides case management support to help seniors remain independent in the community. Services are based on the needs of the individual, but could include: a needs assessment, assistance with applying to government programs (Social Security, HEAP, Medicaid, Medicare), assistance in managing finances, locating and applying for housing, assistance with medical appointment/prescription access, arranging transportation, end of life planning, making linkage and referral to other needed services, supportive counseling and problem solving.

Program provides assistance for older individuals who seek to maintain independence and live in their own homes or apartment.

Provides system of care for Franklin County older adults to assist them in remaining independent. Provides information, advocacy and case management. Will link older adults to resources such as homemaker services, personal care, caregiver relief, emergency response systems, home delivered meals, medical transportation, adult day services and minor home repair.

Assists public assistance recipients and low income individuals and families in obtaining social services.

Provides specialized services for individuals with a dual diagnosis of mental illness, substance abuse and/or developmental disabilities. Acts as a liaison between the Franklin County Board of Developmental Disabilities and the Franklin County ADAMH system to coordinate services ensuring continuity of care for the client.

An intensive case management program for families seeking to build stability and exit generational poverty. Families are linked with a case manager who supports the family in stabilizing housing, increasing earning power, and helping children thrive in school. Average program length is 18 months. Throughout the program, families can receive financial assistance to facilitate goal achievement.

Provides services for patients with AIDS.

The CARE Clinic offers medical evaluations of children 18 years old and younger where child abuse is suspected or alleged.

Provides anonymous and confidential HIV testing. Provides comprehensive case management to people living with HIV/AIDS. Services include emergency financial assistance, referrals for medical, dental, legal, and other community resources, counseling, support groups, activities and rental and utility assistance. Outreach through Ohio AIDS/STI Hotline.

Provides case management for any minor victim of domestic minor sex trafficking. Also conducts outreach and education in attempts to prevent sex trafficking.
