Provides a range of home health care services including but not limited to skilled nursing services for diabetic and cardiac care, as well as assistance for older adults and those with a chronic illness or disability. Offers personal care and homemaker assistance, including bathing and grooming, light housekeeping, laundry, assistance with daily living tasks, companionship, meal planning and preparation, errand running, and transportation/escort to places such as medical appointments, shopping, and family functions. Also provides additional support, such as rehabilitative therapies and social work services.

A home care program that assists low income persons (age 60 and older) who are eligible for nursing home placement to remain in home in their community.
