Offers a daytime program for older adults which includes, limited available transportation, one meal, snacks and activities, such as arts and crafts, day outings, exercise, and musical programs.

Offers structured activities in a supervised adult day care environment for memory impaired and physically frail adults ages 62 and up. Accepts patients with Alzheimer's whose diagnosis is mild to moderate. Transportation is available within a ten mile radius of the center and lunch and snacks are served.

Offers a therapeutically oriented, ambulatory day program to military veterans only. Provides health maintenance and rehabilitative services to frail individuals in a congregate setting. Lunch is provided.

Provides a Medicaid-funded program that offers home health care and services coordination. Services include personal care, homemaking, emergency response system, home-delivered meals, adult day services, medical transportation, counseling services, medical equipment and supplies, and long-term care planning.

Adult day service centers provide a coordinated program of professional and compassionate services for adults in a community-based group setting. Some general services that will be offered are: - Social activities: interaction with other participants in planned activities appropriate for their conditions - Transportation: door-to-door service - Personal care: help with hygiene, grooming, eating and other personal activities of daily living - Therapeutic activities: exercise and mental interaction for all participants.
