TeleHealth Monitoring Unit is placed in patient's home and will measure blood pressure, weight, blood sugar, oxygen concentration on a daily basis (or more often as needed). Will also remind patient to do measurements at prescribed times. Unit is monitored by Interim Healthcare Nurse by computer at office. Nurses can be sent out to do medical intervention as ordered by physician.
