The Community Care Coordination program locates, activates and coordinates a variety of in-home services needed by individuals aged 60 and older who are at risk of entering a nursing home and who are not eligible for PASSPORT.

Provides nursing aide care, respite services, and home health care. ACMC Regional Home Health is a Medicare and Medicaid certified Home Care Agency.

Each Medicaid waiver offers different services including long term home health care.

Provides full range of outpatient services for the home bound; includes certified and private duty services.

Provides skilled home health care services to residents of Montgomery County. Services include: nursing; physical, occupational, and speech therapy; nutrition; enterostomal therapy; intravenous (IV) therapy; social work; and home health aides.

Provides home health services to the elderly, ill and disabled. Services include: bathing and tuck-in services, companion care, Handy-Helper, respite care, nutritional counseling, transportation services, housekeeping, telephone reassurance, and bath and breakfast services. Services available for acutely ill include: home health aides, physical therapy, speech therapy, occupational therapy, and IV therapy.

A home care program that assists low income persons (age 60 and older) who are eligible for nursing home placement to remain in home in their community. Long term home care services available include skilled nursing care, home health aides, medical social services, wound care, IV (intravenous) care, personal care, physical, occupational and speech therapy, and chronic disease management.

PASSPORT provides long term home health care for older adults as alternative to nursing home care. Licensed, professional PASSPORT staff members assess clients' needs, make recommendations, and arrange for services which are provided in-home through contracted agencies.

Provides services to older adults in their homes as an alternative to nursing home care. PASSPORT services may include personal care/respite, homemaker, adult day care, home delivered meals, chore service, minor home modifications, adaptive devices, home medical equipment, pest control services, transportation, social work, counseling, emergency medical response systems, independent living assistance and community transition services.

Provides help to older adults who want to remain independent and living in the community. Enrollees receive personalized services that are coordinated by a care manager. The care manager can assist with things like disease management, health insurance benefits, service coordination and transitional care. Services may include adult day care, chore, homemaker, home delivered meals, personal care, safety monitoring and minor home repair.

PASSPORT offers long term healthcare.

Provides a Medicaid-funded program that offers home health care and services coordination. Services include personal care, homemaking, emergency response system, home-delivered meals, adult day services, medical transportation, counseling services, medical equipment and supplies, and long-term care planning.

Provides: home health care (home health aides and home nurses) by medical staff; homemaker services; and durable medical equipment sales and rentals.

Provides home health care, personal care, and homemaking services to individuals ages 60 and up.
